Job Description:
This company is looking for a Care Coordinator to assist their Latham office. This person will be working with various members within the office (nurses, scheduler, directors, etc.). This person will be communicating with patients with various coordination needs. This could include:
- Collecting data regarding previous services
- Care coordination
- Scheduling in home visits from company Nurses and Physicians
- Confirming patient information is correct in company computer system
- Other administrative duties as needed
- The care coordinator will be constantly communicating both internally and externally with workers and patients.
