Licensed Practical Nurse

This position is responsible for the provision of direct client care and interaction under the supervision of a
Registered Nurse (RN), Nurse Manager or physician. The LPN provides medication administration, wound
care, patient and family education and historical data collection. Documents care provided in the clinical
record. The population served includes adults. OSHA Statement: This position has potential for exposure to
blood and body fluids. 

Medications prn's and scheduled, orders, therapeutic activities - ensures completion, shift note, medication
inventory and ordering (night shift), provide input to physician, obtain patients for MD, peer evaluations to
assist Nurse Manager/Charge RN.

Clinical Processes & Procedures
 Ensure that resident’s charts are updated and free of errors.
 Openly communicate with the Medical Director and D.O.N. relating to resident diagnoses and
treatment plans.
 Collaborate with the clinical team in establishing a treatment plan.
 Provide direct resident care following the treatment plan, this may include tasks such as administering
medication, etc.
 Staffing
 Makes walking rounds between shifts and whenever necessary to supervise and monitor work
performance in terms of resident care, staff regulations and efficiency of service.
 Survey Readiness
 Report occupational exposures to needle sticks, blood, body fluids, infectious materials, and hazardous
chemicals in accordance with facility policy and procedures governing accidents and incidents.
 Participates in all inspections of nursing care by outside agencies
 Complete all documentation as recommended by Federal and State Guidelines.
 Ensure that reportable events are communicated in a timely fashion.
 Ensure that nursing notes are informative and complete regarding the cares and services provided to
the resident.

 Unit Management
 Listen to shift report and keep open lines of communication with supervisor, co-workers and on-coming
shift.
 Attend nurses and team meetings as scheduled and/or assigned.
 Maintain professional relationships with physicians and other providers.
 Respond to all emergency situations within the facility.
 Promote rehabilitation nursing on the units.
 Perform physical and psychological nursing assessments on residents when necessary.
 Report all unusual circumstances that may involve the safety and well-being of the residents and staff
directly to the D.O.N.
 Performs other duties as assigned.

Physical Requirements:
 No Prior Experience Required
 Excellent communication skills at level necessary for understanding and relaying instructions to
participants/patients and for accurately documenting information.
 Ability to deal tactfully with personnel, patients, family members, visitors, government
agencies/personnel and the general public.
 Punctual
 Effective time management skills
Certification /Educational Requirements:
 Education-Successful completion of a Licensed Practice Nurse (LPN) program at an accredited school,
college or university.
 Previous experience as an LPN in long-term care preferred. At least 1 year.
 Certificates, Licenses, and Registrations
 Must have and maintain an active LPN license / registration in good standing on the state registry.
 CPR certified.
 Availability & Attendance
 Flexible work schedule (mornings, afternoons, nights, weekends, holidays, etc.) which may include
hours beyond eight hours in a day, or 40 hours in a week.
 Follows all attendance policies and procedures, including punctuality and proper time clock. usage.
 Compliance with infectious disease policies and procedures related to, but not limited to tuberculosis
screening of health-care workers.
 Excellent knowledge of medical and hospital terminology
 Good knowledge of MS Office and data entry
 Perfect physical condition and stamina
 Excellent communication and interpersonal skills
 Compassionate and able to handle stress

 

Registered Nurse 

Organizes, directs, and oversees the work of the nursing staff, nursing programs, and compliance with facility
policies and procedures to ensure the highest degree of quality care is offered. Provides skilled nursing care
and services to residents. Supervises staff as assigned in the day-to-day delivery of resident care. Such
supervision must be in accordance with state and federal regulations and as may be required by the Director of
Nursing or Designee to ensure the highest degree of quality care is maintained.

Duties and Responsibilities:
• Coordinate and participate in resident care.
• Manage and administer IVs.
• Supervise, instruct and assist LVN/LPN Nurses, Treatment Nurses and CNAs in provision of care with
prompt response to call signals.
• Make rounds and provide report.
• Perform skilled care assessments.
• Participate in orientation and on-going training of staff.
• Respond to and monitor care issues and changes in condition.
• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Supervise staff, including taking or assisting with appropriate disciplinary measures.
• Assist physicians and other healthcare providers when making visits and conducting examinations.
• Communicate with physicians and other health professionals regarding resident care, treatment and
changes in condition.
• Communicate with resident families/significant others regarding care.
• ·Coordinate preparation for new admissions.
• Coordinate and respond to medical emergencies.
• Communicate new or updated orders for rehabilitation therapies.

• ·Verify laboratory test samples are obtained as ordered, results are received timely and physicians are
notified of results promptly.
• Provide resident care including carrying out physician’s orders for care, providing medication and
treatment.
• ·Report significant findings or changes in condition and potential concerns to Director of Nursing.
• Participate in development, maintenance and implementation of the facility’s quality assurance
program for the nursing department.
• Participate in facility surveys (inspections) made by authorized government agencies as requested by
Administrator, Director of Nursing or Assistant Director of Nursing.
• Oversee direct care activities to assure care delivery is consistent and follows policies and procedures.
• Oversee the development and implementation of care plans. Review for regulatory compliance and
nursing standards.
• ·Assure physician orders are current, accurate, signed, and communicated to the nursing staff and
other clinical disciplines.
• Assure documentation and investigation of incidents are appropriate and monitor for accuracy and
completeness.
• Facilitate interdisciplinary communication and collaboration.
• Collaborate with ancillary services for efficient, timely, and accurate delivery of services and reports
(Pharmacy, Labs, Therapies).
• Regularly communicate information and changes to staff.
• Counsel, coach, and educate employees to maintain or improve performance.
• Complete audits of the med carts, med rooms, charting/documentation and other nursing practices.
• Monitor and evaluate infection control practices.
• Participate in new employee orientation and trainings.
• Identify staffing needs.
• Be ready and willing to function in all areas of direct patient care that your license supports.

supervising assigned team members.
• Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care
goals and standards; making or recommending necessary adjustments; following hospital and nursing division's
philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing
agency regulations.
• Protects patients and employees by adhering to infection-control policies and protocols, medication
administration and storage procedures, and controlled substance regulations.

Certification /Educational Requirements:
• Education-Successful completion of a Registered Nurse (RN) program at an accredited school, college
or university.
• Previous experience as an RN in long-term care preferred. At least 1 year.
• Certificates, Licenses, and Registrations
• Must have and maintain an active RN license / registration in good standing on the state registry12
months experience in clinical nursing and an additional 6 months’ experience in providing nursing care to
patients on maintenance dialysis. Certain state specific regulations may apply.
• 1-year previous dialysis management experience preferred.
