Patient Care Coordinator Job Responsibilities

  • Knowledge and ability to collect weekly schedules from each clinician each Friday for the following week.

  • Evaluates referrals to determine if patients meet Medicare eligibility for home health services

  • Enters referral information into DeVero system, including Intake orders, patient's demographics and medical course of treatment

  • Ability to review caseload per clinician for the previous week. Documenting frequency.

  • Knowledge of patient documentation discharges/ recertification

  • Knowledge to identify payer information and verify home health coverage

  • Ability to obtain home health services as required

  • Ability to communicate professionally orders and services to patients/families

  • Knowledge of patient recertification

  • Knowledge of documentation any Hospitalizations and Transfer Note is in from the clinician.

  • Experience scheduling clinicians to provide appropriate care

  • Ability to maintain clinician's calendars

  • Ability to follow up with hospital case managers to ensure that patients appropriately transition back to hospice/home care.

  • Ability to maintain and keep up with required hospice and home health authorizations with HMO's.

  • Ability to follow up with hospital/ SNF to see when patient will be D/C.

  • Knowledge of obtaining orders once patient is D/C so clinicians can resume care.

  • Knowledge to follow-up on missed visits and confirm with clinicians of all upcoming visits scheduled.

  • Experience with DME- New referrals place orders, and new requests.

  • Maintaining the On-Call Calendar and endorse to Intella Triage on a monthly basis/update Intella Triage of any changes to On-Call calendar

  • Collects and maintains statistical data on all referrals and admissions and reports information daily, weekly, monthly, and as requested.

  • Maintain and keep up with Required Hospice Authorization with HMO's.

  • Prepare IDG Agenda and re-certifications.

  • Performs other tasks as assigned

Customer Service

  • Answers telephone lines promptly and efficiently. Responds to message content appropriately; disseminates information to the appropriate persons.

  • Surveys patients/families to determine level of satisfaction with agency services. Documents findings and notifies appropriate supervisor of any dissatisfied customers. Must complete one survey per business day.

  • Scheduling Start of Care visits and or/ communicating with patients & clinicians in a timely manner.

Organizational Responsibilities

  • Prioritizes work effectively and is acceptably productive.

  • Maintains an acceptable attendance record according to agency policy.

  • Accepts responsibility for behavior and activity.

  • Participates in agency's quality improvement program.

  • Interacts collaboratively with all team members.

  • Maintains confidentiality of patient and agency information at all times.

  • Back up assistance in office as needed.
