Home Health Aide

We are looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services of residents of Brooklyn Adult Care Center in accordance with an established care plan.

Responsibilities

• Provide health care services in the facility
• Perform personal care tasks
• Accompany patients to doctors office, on-site and off-site
• Assist with clients personal care activities
• Monitor patients and report on their condition to nurses
• Reporting and documenting of patients care and provided services
• Assist patients with ambulation and transferring
• Provide companionship and basic support

Skills

• Proven working experience as home health aide
• Ability in following basic care instructions from supervisors and nurses
• Proficiency in English; other languages, a plus
• Caring and compassionate personality
• Able to work in a team
• Current HHA certificate
• Valid government ID

Job Types: Full-time, Part-time

Salary: $15.00 /hour
