*JOB DESCRIPTION:*
A LPN promotes the residents' health, wellness and illness prevention through daily nursing and personal care endeavors. A LPN works under the direction of a Registered Nurse and handles a diverse range of tasks including, administering treatment or medications, performing clinical procedures and documenting the clinical data of patients, among other duties.
*RESPONSIBILITIES:*
· Ensures the care and safety of all residents in the nursing unit
· Ensures that individual care plans are followed
· Administers and documents medications, treatments, and other nursing services
· Makes a count of controlled substances on the assigned nursing unit
· Records current condition of assigned resident in EMR
· Receives and gives an accurate report of the resident condition at shift change
· Communicates to the nursing team and medical provider as indicated and required
· Assists in the orientation of new staff
· Maintains competency in all the tasks needed by CMS
*REQUIREMENTS:*
· Minimum 18 years of age
· New York State Licensed Practical Nurse licensure (LPN)
· Strong written and verbal communication skills; able to follow written and verbal instructions
· Basic computer knowledge, PCC e-MAR experience preferred
· Physical endurance for frequent activity; ability to lift a minimum of 20 pounds
