Full-time Palliative Care Telehealth Opportunity

Monogram Health is seeking a board-certified Palliative Care physician in New Jersey (Newark, Trenton, Cherry Hill).  The physician would be expected to manage patients who are on our conservative pathway. The Monogram IM will hand off care to the palliative care physician at the chosen time. The Palliative Care physician will follow the patient as part of their panel and will be expected to manage the patient to a conservative pathway. This will involve addressing daily issues via phone, as well as in-person visits. 

Monogram physicians are quality-driven. They apply their clinical expertise in a managed care, population health context. They promote palliative care where clinically appropriate, patient education, autonomy, and quality of life issues. This job is ideal for physicians who are forward-looking and embrace value-based care. Qualified physicians must be board-certified in palliative care or geriatrics and must be currently licensed in the state of New Jersey.
