Provide direct patient care assistance in the patient's residence under the direction and supervision of the RN and according to the Plan of Care
Effectively communicate with the Interdisciplinary Team to ensure patients and their families receive the alleviating care, comfort and compassion they deserve
Perform or assist patient/family with home safety and activities including, but not limited to, light housekeeping and assistance with meals
Appropriately document and monitor adequate nutrition intake, temperature, pulse, respiration, skin and basic elements of body functioning and changes
Provide compassionate communication and emotional support to the patient, family and other employees
Assure for compliance with local, state and federal laws, Medicare regulations and organizational policies and procedures
Complete annual educational requirements
