Job Summary and Qualifications

The Nurse Navigator for Southern Hills Hospital & Medical Center’s Orthopedic and Spine Program facilitates care for patients by serving as a liaison throughout the continuum of care. This is achieved by anticipating and addressing patient needs before, during, and after their hospitalization, in consultation with the medical team, to include the patient’s surgeons and the Orthopedic and Spine Surgery Team.   • You will conduct data review to identify trends in patient experience and complication rates and assist in implementing strategies to achieve optimal care for future patients.
  • You will serve as a patient advocate for all patients and provide support to navigate the healthcare system.
  • You will provide appropriate teaching, outreach, and education to patients and families.
  • You will conduct follow-up conversations with all patients and communicate concerns, changes or social needs in patient health to the appropriate care providers. 
  • You will drive process improvement and deliver reports to demonstrate program outcomes and support performance improvement activities.
What qualifications you will need:
  • Bachelor’s Degree in Nursing from an accredited program required, Masters in Nursing or related field preferred
  • Current licensure as a Registered Nurse in the state of Nevada
  • Current BLS certification required (American Heart Association or American Red Cross)
  • Minimum two (2) years' Ortho/Spine clinical nursing experience highly preferred 
  • Orthopedic Nursing Certification, if not already obtained, will be required to be obtained within one year from date of hire
