Job title: Housing Case Manager

Job description:

The position is full-time. The Housing Case Manager will support the culture of a community-based, person-centered, and trauma-responsive program. The role will focus on assisting individuals who have experienced homelessness and barriers to living in the community to live successfully in permanent supportive housing. The position assists with housing assessments connecting residents to community resources and includes ongoing community support, case management services, and crisis intervention. Assist with mental health assessments, treatment/discharge planning.

Job responsibilities:

  • Coordinate and provide linkage and referrals to other community supports and services. Advocate on behalf of members and make recommendations as needed. Intervene and advocate with collateral sources, agencies, bureaucracies, facilities, and families on member’s behalf.
  • Provide community support services and assist members in achieving and maintaining rehabilitative, resiliency, and recovery goals. Services provided include therapeutic interventions that facilitate illness self-management, skill-building, identification and use of natural supports, and community resources.
  • Assist members with housing needs (e.g., transitioning in and out of apartment program, developing skills necessary to live independently).
  • Provide crisis intervention when necessary
  • Conduct individual and group therapy/ counseling with members and their families; this will include facilitating group, writing group curricula, maintaining group.
  • Perform outreach and home visits.
  • Provide transportation to members of the Independence Center following all transportation policies and protocols.
  • On-call duties.

Requirements:

Requirements

  • Candidates with solid housing expertise ideal.
  • Candidates with solid knowledge of mental health and substance use disorders desired.
  • Experience working alongside diverse populations.
  • A Bachelor’s degree in human services or a related field. Masters Degree Preferred.
  • Bilingual preferred.
