JOB SUMMARY

The Patient Safety Officer is responsible for forward facing initiatives around Patent Safety and organizational quality. The PSO is responsible for leading organizational programs as well as departmental projects and teams that support The Joint Commission standards and CMS conditions of participation. The PSO assists with a proactive response to patient safety by recommending changes, policies or initiatives that could prevent future errors.

MINIMUM QUALIFICATIONS

Education: Bachelor’s degree in area of practice.

Experience: 5 years of professional experience in hospital or healthcare setting.

Licensure/Certification: Current West Virginia License in area of practice. CPPS certification within 1 year of hire.

JOB SPECIFIC FUNCTIONAL SKILLS

Participates as a member of the Medication Administration Committee and supports the activities and projects approved by the Medication Administration Committee.

Responsible for addressing potentially preventable events or risks related to medication safety practices.

Develops National Patient Safety Goals policies and oversees education and compliance.

Assures the Patient Safety Plan is compliant with regulatory standards annually.

Provides bi-monthly summaries and the annual report to the Quality Board on Patient Safety.

Provides organizational orientation and education on patient safety.

Develops and implements key indicators and measures to monitor patient safety improvements. Maintains the Patient Safety Scorecard.

Facilitates the selection and provides leadership in conducting proactive risk assessments including the Failure Mode and Effects Analysis.

Analyzes patient safety data and recommends/implements proactive solutions.

Serves as a facilitator in the Culture of Safety and assists with the administration of the Culture of Safety Survey at routine intervals.

Leads the facilities continual regulatory readiness efforts and participates in regulatory body surveys.

Coordinates Sentinel Event Alert responses for all Joint Commission alerts including collaboratively working with interdisciplinary teams to identify current practices and implement new practices recommended by The Joint Commission and document response.

Serves as a back up to the Patient Advocate and assists with complaint/grievance documentation and resolution as needed.
