West Virginia University Research Corporation is seeking applications for a Hybrid (Part-time) ACE Program (WVU Drug Free Moms and Babies) Project Coordinator with the Department of Obstetrics and Gynecology

About the Opportunity

The Assist, Connect, and Encourage (ACE) Program is a Drug Free Moms and Babies (DFMB) Project Site at WVU Dept of OBGYN that provides additional care coordination and support for individuals with substance use disorder in pregnancy and after birth. The ACE Program is located at the Morgantown and Bridgeport/Fairmont campuses. This position is for the Morgantown location providing services to those receiving prenatal and postpartum care at clinics affiliated with WVU Children's. The Project Coordinator is a crucial member of our small team. The Project Coordinator provides direct coordination of care for ACE clients through; comprehensive assessment, referral to services and treatment, development of individualized care plans, care coordination, health education and promotion, and monitoring and follow up. They also assist in obtaining necessary consent for program participation and data collection for program evaluation. The Project Coordinator helps to promote the program through community outreach and is crucial in completing and maintaining enrollment of clients for the Maternal Opioid Use Model. This position is hybrid with telework being a large component. A work phone and laptop are provided to the Project Coordinator to help fulfill this role.

Accountability: DFMB Project Coordinator shall be employed by qualified DFMB sites as defined in the Provider Enrollment and Participation Requirements section of this policy and able to fulfill the in-person, virtual, and telephonic care coordination activities defined in DHHR Chapter 521 Appendix B. They will work under direct supervision of the health care professional or provider at the health care center.

What you'll do:

  • Work closely with principal investigator to direct and manage Drug Free Mom and Baby (DFMB) / ACE (Assist, Connect, Encourage) services, staff members, budget, and grant requirements.
  • Assist the client in navigating the healthcare system- serve as the liaison between the client, OBGYN, and Behavioral Medicine.
  • Serve as the point of contact to the recovery coach and provide support by assisting the recovery coach in identifying and resolving problems and making effective use of resources.
  • Provide support for the SBIRT model within OBGYN department, including monitoring screening process, meeting with patients for brief interventions when appropriate, and encouraging motivational interviewing techniques.
  • Receive and process program referrals, including reaching out to referred clients and completing enrollment process and forms.
  • Link clients to available resources including, but not limited to; continuing education/professional development, safe housing, recovery supports, self-help groups, medical/health facilities, and substance abuse or mental health services.
  • Serves as the representative for the DFMB Program with institutional, local, and state organizations.
  • Maintain working relationships with DFMB and MOMS grant staff and other agencies or institutions in order to provide awareness of the availability.
  • Maintain outreach materials (both print and electronic outlets) and conduct outreach as necessary for program promotion.
  • Document client progress in the program and interaction with staff members for program monitoring and evaluation.
  • Assist in the necessary enrollment forms and documentation for MOMS requirements and, eventually, for Medicaid reimbursement for services.
  • Collect and maintain Perinatal Partnership required data and document in the REDCAP system for program evaluation.
  • Independent frequent travel to various OBGYN clinics affiliated with WVU.
  • Attend scheduled staff meetings and trainings, as required. These typically include: the annual WV Perinatal Summit, monthly ACE Program meetings, monthly DFMB meeting, and weekly touch-base with Peer Recovery Support Specialist.
Qualifications
  • Bachelor's Degree in Sociology, Psychology, Counseling, Healthcare related field
  • Two (2) years of experience:
    • Hands-on work experience with project and patient care coordination preferred.
    • Previous work with diverse populations of families, preferably with social use disorder history preferred
    • Previous work providing customer service or patient care is preferred
  • Knowledge of treatment resources, including a working knowledge of recovery and perinatal resources.
  • Ability to comply with WVUH, WVU Research Corporation, UHA, and Department of OBGYN safety procedures.
  • Completion of HIPPA training and compliance (to be completed upon hire).
  • Skilled at working as a team member.
  • Skilled at written/oral communication.
  • Knowledge of basic computer applications, including Word and Excel.
  • Excellent customer services skills and the ability to communicate effectively with patients and families; pharmaceutical company personnel; regulatory and monitoring agency personnel; and internal staff members. Communicates with physicians, patients, students, and employees of WVU, WVU hospitals, and UHA.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public.
  • Ability to develop constructive and cooperative working relationships with others and maintain them over time.
  • Ability to analyze information and evaluate results to choose the best solution, solve practical problems, and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to develop specific goals and plans, to prioritize, organize, and accomplish tasks independently and through others.
Requirements

The ability to travel to WVUs regional campuses and/or within the multi-state area is required.
