1. Overview
1.1 Results for Development Institute (R4D): Results for Development (R4D) is a leading non-profit global development partner. We collaborate with change agents around the world - government officials, civil society leaders and social innovators - to create strong systems that support healthy, educated people.
is a global initiative funded by the U.S. Agency for International Development (USAID) that provides catalytic support to countries as they tackle vexing health systems challenges and accelerate progress toward self-reliance. The Accelerator is led by R4D with support from Health Strategy and Delivery Foundation (HSDF) and ICF.
In Ghana, the HSS Accelerator is supporting the Government of Ghana (GoG) and other local actors to ensure adequate and efficient use of health sector resources to achieve Ghana's vision of health for all.
1.2. Program Overview:
Ghana's Universal Health Coverage (UHC) roadmap, which aims at ensuring every Ghanaian has access to an essential package of health services by 2030, identifies Networks of Practice as a key strategy to achieve this goal. The pilot of Provider Networks in the Volta Region recommended the need to ensure that network hubs are well resourced in terms of HR capacity, equipment, and infrastructure. In line with this recommendation, the Ghana Health Service has re-designed the concept of network implementation in Ghana with the introduction of the 'Model Health Center' as the network hub. The network hubs serve as the main referral point for sub-district level facilities within a network.
The Accelerator with support from USAID seeks to support GHS to enhance the capacity of the model health centers (Network hubs) in the pilot districts by equipping these facilities with basic medical and laboratory equipment to enable them function as referral points within the networks.
1.3. Purpose and Objective:
As part of support to the GHS, The Accelerator is looking for a vendor(s) to supply the underlisted medical and laboratory equipment. In addition, the vendor(s) will be expected to conduct site visits, install equipment, and build capacity of facility users. The vendor will also allow adequate time for testing of the equipment before signing off on completion of the contract.
1.4. The Opportunity
Price Quotation is requested for items outlined in tables 1 and 2 below. Detailed minimum requirement specifications are described in the link at Appendix B.
The offeror has the option to provide quotes for either medical equipment or laboratory equipment or both according to their capacity.

1.4.1 Specific Requirements

  • Vendor will be required to carry out the following tasks:

  • Conduct a tour of six health centers located in South Dayi and South Tongu districts in the Volta region.
  • Advise on capacity of health centers in terms of adequacy of electrical power supply systems (national grid, back-up systems, especially Photovoltaic installations) availability of space for installation especially laboratory equipment, capacity of staff to use and maintain equipment and make recommendations on modifications or otherwise as necessary.
  • Supply equipment based on specifications and quantities agreed on by the GHS/Accelerator and offeror following inspection of health centers.
  • Install and setup equipment.
  • Enhance the technical competencies of GHS staff on the use, maintenance, and installed-base management of the equipment to ensure sustainable deployment of the equipment.
  • Provide warranty and 2 years post warranty maintenance and support services.

1. General Information

2.1. Proprietary Information
All information contained in this RFQ is proprietary. It is for the sole use of the Offeror in connection with bid response preparation and is not to be used by Offeror for any other purpose or revealed or disclosed in any manner to any other individuals and/or company without specific written permission from R4D.

2.2. Procurement Protocol
R4D reserves the right to make all decisions regarding this RFQ, including the right to reject any and all quotes received in response to this RFQ. Offeror understands that the issuance of this RFQ does not create any obligation on the part of R4D to enter into any contract or to undertake any other obligation with respect to the requirements referred to herein.

2.3. Addendum
In the event that any modification to this RFQ becomes necessary, Offeror will be notified via email.
2.4 Notification of Supplier Selection
Offerors will be notified via email of the results of the selection process.

2.5. RFQ Response Preparation Cost
All costs incurred by Offeror in the preparation of its response to this RFQ, and any oral presentations requested, shall be wholly absorbed by the Offeror.

2.6. Confidentiality/Proprietary Information
Materials submitted by Offeror in response to the RFQ that are considered confidential/proprietary must be clearly marked as such at the time of submission.

2.7. Contract Terms and Conditions
Engagement with selected Offeror will be based on R4D's standard terms and conditions. It shall constitute the written agreement between Offeror and R4D and shall not be binding until fully executed and delivered by both parties.

2.8. No Publicity
No publicity or news release pertaining to this RFQ, responses to this RFQ, or the award of any contract related to this bid document may be released without R4D's prior written approval.

2.9. Accuracy of Information
This RFQ and other bid documents furnished by R4D may include errors, omissions, or deficiencies and the accuracy and completeness of such documents is not guaranteed. In the event of such errors, omissions, or deficiencies are discovered by Offeror, it shall promptly notify R4D in writing. Corrected information will be provided to all Offerors.

2.10. Alternative Solutions
Offeror(s) who wish to submit an alternative proposal in addition to the requested RFQ response are invited to do so. Alternative solutions must be clearly marked as such and state whether they are exclusive or inclusive of the requested RFQ requirements.

2.11. Third Party Services/Products
Offeror must clearly identify any third-party product(s) or service(s) that is a part of its RFQ response. Evidence of bidder's relationship with original manufacturers should be provided, in addition to a manufacturer's authorization to distribute and provide technical support.

2.12. Manufacturer Restrictions
Offers must not include equipment manufactured by companies (or any subsidiary or affiliate of such entities) restricted by the US Government. All products shall be CE marked (From Europe and the UK), and FDA-approved (from the USA)

2.13. Offeror's Response
A clear and complete response must be provided for each element of the RFQ. Any response which deviates from the stated requirements must be noted and underlined.

2. Communications Regarding this RFQ

3.1. Intention to Bid and Quote Delivery
Offerors must submit an offer to this RFQ no later than 26th January 2024, and are allowed to make enquiries about the RFQ no later than 19th January 2024.

It is our intent that the process be expeditious, concise, and straightforward. If, at any time during the process Offeror should elect to discontinue pursuit of this opportunity, you are requested to notify us via email to the addresses above.

3.2. Contractual Obligations
Offeror's response to this RFQ, in the whole, in part or by reference, may be incorporated into any resulting contractual agreement at the sole discretion of R4D to ensure that all representations made by the Offeror are legally binding.

4.2. Evaluation Criteria
Offeror responses to this RFQ may be evaluated on criteria such as, but not limited to, the following:
Business Experience and Stability: Offeror demonstrates the experience and capacity to supply similar items in terms of quality and quantity as well as demonstrated ability to provide spare parts and maintenance.
Completeness of Service Proposal: Offeror demonstrates an understanding of the request and how to meet it effectively and efficiently
Pricing/Cost: Offeror provides quotes that are competitive but realistic for both supply and post-supply requirements.
Customer References: Offeror provides clients they have or still serve with satisfaction
Delivery and Payment terms: Offeror provides the most advantageous delivery schedule and most flexible payment terms that include post installation period for equipment testing.
Warranty: Offeror provides the most attractive warranty in terms of durations and services
Evidence of ISO certification as a medical equipment distributor, especially for laboratory and diagnostic equipment as part of Quality Assurance for technical service provision capabilities

5. Organization of the Proposal

  • R4D will be thorough and comprehensive in our deliverable and service demands. It is incumbent upon the Offeror to provide measurable, accurate, reliable, and well-organized response based on our predefined evaluation criteria. We will rely on the quality of your response. A forthright approach will serve all parties best. To this end, you should note responses may be incorporated within the eventual contract with the selected Offeror, at the discretion of R4D.

A clear, definitive response to each element of the RFQ must be included in the Offeror's proposal. Any exceptions must be noted, typed in all caps and underlined. Failure to address any stated requirements/specifications may disqualify your proposal from further consideration.

Responses should be concise and contain all the required documents in a zipped folder bearing the name of the offeror and sent only via email. Responses should contain documentation on the sections below:

5.1. Letter of Transmittal
  • A transmittal letter must accompany all proposals. This letter must be prepared on the Offeror's letterhead, signed and dated by an authorized officer or partner of the submitting firm who has the authority to sign and legally bind the Offeror. The title of the individual signing the proposal, the legal company name must be included with the signature. The transmittal letter must contain the following information:
  • Outline of the quotation
  • Clear identification of any RFQ requirements the Offeror cannot satisfy.
  • A statement describing the basis upon which the Offeror is qualified to address R4D's requirements as outlined in this RFQ.
  • The names and positions of individuals authorized to negotiate with R4D on behalf of Offeror.

5.2. Service Provider Profile
Offeror must provide the specific information requested in the Service Provider Profile contained in Appendix A.
5.3. Demonstration of Offeror's Capability
Furnish information on the company's specialized experience and capabilities as it relates to this RFQ, including experience working with non-profit organizations if any.

Describe your client services and how your client services work with customers to ensure optimal level of service delivery and customer satisfaction.
5.4. Service Proposal
Offeror must submit a service proposal outlining services proposed to accomplish each section of the work to be performed. Plan must include, at a minimum, the following components as applicable:
  • Description of items
  • Detailed plan to meet request
  • Timelines
  • Personnel requirements
5.5. Pricing Proposal
Offeror shall provide a breakdown of costs, in alignment with the specified units. Breakdown shall include as applicable:
  • Product name
  • Quantity
  • Price in GHS, period validity for price quotes
  • Terms and conditions (including delivery and payment terms)
  • Annual maintenance costs
  • Start-up consumables, and a six month-supply of a seed stock
  • Cost of mandatory and optional accessories and software licenses and periods for renewal
  • Accepted mode of payments
  • Etc

5.6. Minority and Women Owned and Small Business Enterprises
R4D has been and continues to be a strong advocate of diversifying its supplier base and increasing our volume of business with minority, women owned and small business enterprises. Indicate whether or not your company has a MWBE certification by an accredited certification body (i.e. National Minority Development Council, Small Business Administration, Women's Business Enterprise National Council, etc.). If certified, provide name of certifying agency and date your current certification expires.

6. Appendices

6.1 Appendix A. Service Provider Profile (As applicable)
Please provide the following specific background information about your company and the services that you provide as applicable:
Company profile:
  • Brief history of your company
  • Location of your offices/headquarters
  • Full company or corporate name and employer identification number/registration number (as applicable), such as DUNS#.
  • Type of legal entity (corporation, partnership, etc.)
  • Date of incorporation
  • ISO certification (added advantage)
  • Describe what feature of your services or organization differentiates you from your competitors
  • Disclose anything that can be construed as a conflict of interest between R4D and the Offeror
Company financials:
  • Your company's credit rating (as applicable) such as Dan and Bradstreet report.
  • Disclose any and all judgements, pending or expected litigation, real or potential financial reversals that might materially affect the viability and stability of the Offeror, or warrant that no such condition is known to exist.
  • Describe three (3) financial references including company, contact name, telephone number and email address.
Company personnel:
  • Indicate the total number of employees in your company
  • How many offices do you have and where are they located?
  • Provide an organization chart. Describe how staff is managed and how each employee participates in the delivery of service to the client. Please provide CVs of technical staff (biomedical engineers, project managers, clinical application trainers etc)
  • List key personnel who will be servicing the R4D account, should your company be the successful Offeror, and provide a brief resume for each.
Company performance:
  • Provide at least 3 client references, including company, contact name, phone number, and email address for which you have provided services similar to those requested herein. R4D reserves the right to contact the references.
  • Indicate whether or not you have ever had a service agreement canceled due to reasons within your company's control, such as poor performance. Provide a general explanation of the incident.
  • Indicate whether or not your company has been the subject of a lawsuit for services rendered. Provide general explanation of the case.
