Insight Global is seeking a pre/post operative registered nurse for an outpatient surgery center located in Charlottesville, VA. The Registered Nurse is responsible and accountable for providing and directing individualized care during the immediate preoperative and postoperative period utilizing the nursing process and established standards of care. This requires assessing, planning, implementing and evaluating the care of patients through clinical knowledge and skills. Will treat all customers with actions that contribute to high levels of customer service.

RESPONSIBILITIES:



1. Selects and prepares appropriate supplies for the pre-operative and post-operative based on the OR schedule, clinical knowledge, and the physicians plan of care.



2. Has a clear understanding of the anesthetic agents and medications used by Anesthesia.



3. Able to recognize, communicate, delegate, and coordinate management of emergent situations in a rapid and effective manner.



4. Provide direct and indirect patient care using the nursing process to assess, plan, implement and evaluate patient requirements as directed by the physician; use medical equipment in accordance with standards; demonstrate continued competence in all facets of ambulatory surgery.



5. Preceding Day:

a. Check the next day's schedule for assignment

b. If unfamiliar with the procedure, read up on it

c. Ensure each patient care area is supplied with necessary supplies and properly functioning equipment.





6. Pre- Operative: Accurately and thoroughly completes and reviews the pre-operative assessment prior to procedure time to include but not limited to:

a. Identification of patient

b. Completion of consent forms

c. Review of chart for completeness

d. Contacts appropriate individuals regarding any abnormalities

e. Assessing of health status of patient through use of history and physical and nursing assessment as related to:

1. Site preparation

2. Nutritional status

3. Emotional status

4. Patient's understanding of post-operative procedures

5. Time of last voiding

6. Time of last oral intake

7. Patient's perception of the procedure

8. Allergies

9. Health aide devices (dentures, etc.)

f. Interviews patient and completes patient history

g. Post-operative teaching instructions to patient and family

h. Documents all the above on appropriate forms

i. Have patient change into patient gown

j. Carries out any pre-operative physician orders

k. Performs peripheral IV puncture if ordered



7. Post Anesthesia Care Unit (Phase 1) - is proficient in assessing the patient's level of anesthesia emergence in accordance with policies and procedures and ASPAN standards by:

a. Receiving report from OR Nurse and/or Anesthesiologist

b. Monitors vital signs and reports initial and any abnormalities to Anesthesiologist

c. Assesses patient's respiratory, cardiovascular, circulatory, and neurological status

d. Maintains oxygenation and patent airway

e. Assessing fluid administration site and appropriate rate

f. Assesses tubes, drains, etc. for patency, drainage amount, character, and position

g. Observe surgical site for bleeding

h. Administers medications safely and accurately

i. Executes physician's orders specified while in unit in a timely manner

j. Implements patient comfort measures as appropriate (medication for pain and nausea & vomiting, position changes, and temperature control)

k. Documents all observations and nursing care provided accurately and thoroughly on the appropriate forms.

l. Prevents cross-contamination through frequent and proper hand washing using universal precautions

m. Discharges patient to Phase 2 according to physician's orders, patient's condition, and standards of care





8. PACU Phase 2: Assesses the post-operative care of the patient in accordance with the patient's condition, policies and procedures, and standards of care by:

a. Receiving report from OR/PACU Nurse or anesthesiologist

b. Monitors vital signs according to policy and procedure

c. Assesses patient's respiratory, cardiovascular, circulatory, and neurological status

d. Assessing fluid administration site and appropriate rate

e. Assesses tubes, drains, etc. for patency, drainage amount, character, and position

f. Observe surgical site for bleeding

g. Administers medications safely and accurately

h. Executes physician's orders in unit in a timely manner

i. Implements patient comfort measures as appropriate medication for pain and nausea & vomiting, position changes, and temperature control)

j. Documents all observations and nursing care provided accurately and thoroughly on the appropriate forms.

k. Prevents cross-contamination through frequent and proper hand washing using universal precautions

l. Provides patient with food based on physician's orders

m. Educates patient and family on post-operative instructions

n. Discharges patient to post-operative area according to physician's orders, patient's condition, and standards of care

o. Does pre and post-operative telephone calls





9. End of Day:

a. Collect all soiled and used supplies and take to decontamination area

b. Take soiled trash, hazardous waste, and linen to linen/trash room; any hazardous trash will be placed in red bags.

c. Restock all the patient areas

d. Put away all extra supplies



10. Maintains working status of instruments and equipment; ascertains that broken and/or missing instruments are properly repaired or replaced.



11. Observes cost containment practices; evaluates and maintains appropriate inventory levels.



12. Respects and complies with decisions made by the Administrator and the Clinical Director; communicates with others in a direct, respectful, and effective manner; will treat customers with actions that exemplify high levels of customer service.



13. Will address and respond to all complaints in a timely manner; will notify Clinical Director of any physician or patient complaints.



14. Will participate in the QI plan by tracking any data in accordance with established guidelines.



15. Actively participates in staff meetings and in services and professional organizations.



EDUCATION: Current Virginia RN license

ACLS required

PALS required



EXPERIENCE/ KNOWLEDGE: Minimum of three years knowledge and experience in a Recovery Room and/or Intensive Care `preferred

Prefer Ambulatory Surgery experience

IV therapy skills required
