Lead/Charge Nurse - Evening Shift Position Description

Our Lead/Charge Nurse (LPN/RN) manages and coordinates the provision of clinical nursing service to residents in licensed and unlicensed areas. Our residents are considered low-acuity. Since we have a lower than average nurse to resident ratio, we strongly encourage our nurses to develop a good rapport with the residents. Job Relationships

Reports to Director of Nursing.
Direct management of LPNs, CNAs and Medical Unit Secretaries.

LPN/RN Duties and Responsibilities

Provides clinical expertise and guidance to insure quality of nursing care to residents and enforces safe work procedures.
Monitors residents condition and reports daily to Director of Nursing. Performs nursing rounds on assigned units.
Provides assessment of residents and arranges for provision of necessary nursing care or other professional services in both licensed and unlicensed areas.
Implements daily staff assignments.
Coordinates the admissions, transfers and discharges of residents, both within the total facility and to outside agencies.
Supervises transcription and verification of physicians orders.
Monitors and provides medication administration and treatment when needed.
Performs various routine nursing procedures as needed.
Participates in scheduling of nursing employees, including staff relief coverage.
Participates in in-service training for nursing services, as assigned.
Monitors medical record charting by nursing staff.
Participates in the orientation, performance appraisal and disciplinary guidance for nursing personnel as requested.
Monitors control drug stock.
Oversees the accurate and timely completion of monthly summaries.
Oversees the accurate and timely completion of incident reports.
Oversees and implements the accurate completion of MARs.
Monitors Medical Record charting for completion and accuracy.
Coordinates the accurate and timely completion of weights and vital signs.
Provides clinical services as defined in the LPN job description.
Performs any duties and/or tasks that may be assigned on an as-needed basis by the Nursing Director.
Responsible for insuring lab requisitions are initiated nightly for early morning lab work completion.
Performs nightly chart audits to insure all orders are accurately transcribed on YAR.
Coordinates emergency care management for all residents campus wide.
Participates in nursing on call rotation.

LPN/RN Typical Physical Demands

Ability to walk up to 20 minutes at a time in order to transverse campus.
Considerable hearing ability in order to perceive the nature of sounds.
Frequent walking, bending, stretching, lifting, pushing, and carrying.
Able to see and read newsprint sized letters in order to make visual assessment of residents.
Possible exposure to: airborne pathogens, blood borne pathogens, odors, toxic chemicals, and fumes.
Ability to lift and maneuver items weighing up to 30 lbs. is required.
Must be able to successfully complete the WorkSTEPS Functional Testing program. This program assists in determining whether applicants/employees are able to safely perform essential job functions.

Minimum Experience

At least three to five years experience in patient care.
At least one year experience in a long-term care setting.

License and/or Certification Required

1. License to practice nursing as an Licensed Practical Nurse or Registered Nurse in the State of North Carolina.
