Description

The Children's Home of Poughkeepsie has been caring for children throughout the Hudson Valley for over 175 years! We strive to empower children and families to achieve optimal health and well being.

We are currently seeking a Per Diem Community Clinical Specialist to work in our Community and Family Treatment Support Services (CFTSS) Program. The Community Clinical Specialist will work on an as-needed basis, with flexibility for evening and weekend coverage. The primary coverage area for this position is Orange County, NY.

Position Summary:
The Children and Family Treatment and Support Services (CFTSS) Community Clinical Specialist will be responsible for the service delivery of the Children and Family Treatment and Support Services in accordance with NYS Standards of Care. These include Other Licensed Practitioner (OLP) services, Community Psychiatric Supports and Treatment (CPST) services, and Psychosocial Rehabilitation (PSR) services.

Knowledge, Skills and Abilities Required:

  • Knowledge in clinical psychotherapy with individuals, families and groups; including assessment, evaluation, and treatment.
  • Knowledge of evidence-based practices and trauma-informed care to serve children and families.
  • Strong verbal communication skills and writing skills.
  • Proficiency in the use of Microsoft Office, Google Suite, and web-based software applications.
  • Commitment to Diversity, Equity and Inclusion.

Essential Functions and Responsibilities:

  1. Assist and support the CFTSS Supervisor and Lead Clinician with assessments and evaluations for Medical Necessity.
  2. Develop the Treatment Plan, including client-centered goals, and deliver service components of CFTSS; specifically, OLP, CPST and PSR.
  3. Meet with children/families based on the frequency, scope and duration noted in the Medical Necessity and Treatment Plan.
  4. Provide clinical psychotherapy; including individual, group and family therapy as required. Service provision to include: specific mental/behavioral health interventions.
  5. Provide solution-focused interventions using evidence-based practices.
  6. Provide crisis avoidance interventions.
  7. Provide strengths-based service planning and psychoeducation to children and families in the community.
  8. Collaborate with families, schools, and other provider agencies to successfully provide a continuum of care.
  9. Participate in weekly individual supervision and monthly group supervision.
  10. Complete ongoing training and professional development as necessary.
  11. Complete documentation within 24-72 hours of service provision.
  12. All other duties as assigned.

Measures of Success:

  • Consistently completes at minimum 20 hours of billable services per week.
  • Completes all documentation within 24-72 hours of service provision.

Schedule: Will be determined by the needs of the department

Qualifications

MINIMUM Qualifications:

  • MSW or MHC Degree
  • 2-3 years' experience working in a human services field, preferably with children and families
  • Valid NYS Driver's License in good standing as determined by the agency

PREFERRED Qualifications:

  • LCSW or LMHC
  • Experience in delivery of services in community based settings

Physical Demands and Work Environment:
This position is in our Community Based Services Department. A majority of the work day is spent in the community providing mental health and behavioral support to youth and their families. Services are provided in a variety of home and community-based settings. The Community Clinical Specialist (CCS) must be available after school hours to accommodate the needs of the youth and families we service. The CCS will be required to safely operate an agency vehicle, and to transport youth in said vehicle while adhering to safe driving practices.
The Community Clinical Specialist should be prepared to participate in provider meetings, which are often held virtually, through a variety of platforms. The CCS must be comfortable using video-conference technology.
