LSF is looking for a talented Assistant Program Nurse who wants to make an impact in the lives of others.

This role will consist of a day shift between Monday - Friday.

Purpose & Impact:

The Assistant Program Nurse works under the direction of the Program Nurse, provides Health Services to children and families. Assists in the identification, coordination, completion of service and evaluation of health or family problems of enrolled children. Maintains awareness of current health services available in the community and works with physicians and other local health care providers to meet the needs of enrolled children. Responsible for assisting with the coordination of health follow-up services to identified children. Documents health/family services that have been provided. Monitors health records at center sites. Participates in the HS/EHS Quality Assurance efforts.

Essential Functions:

  • Reviews children’s files during the enrollment process to identify any health concerns and assess the program’s ability to meet a child’s individual health needs before admitting a child into the program.
  • Evaluate, screen and approve children with identified health needs.
  • Plans and recommends appropriate actions for identified health problems before the child attends class.
  • Once notified by Family & Community Engagement Specialists, ensures that children’s health plan is documented with known health problems.
  • Observes children in classroom setting.
  • In accordance with the Nurse Practice Act, delegates to appropriate center staff the administration of medication and skilled nursing procedures for individual children as appropriate. Monitors the administration and documentation of the individual children.
  • Supervises the administration, storage and documentation of the fluoride administered at assigned centers.
  • Serves as consultant to appropriate staff regarding individual child’s health needs.
  • Establishes an effective schedule of center visits in collaboration with other Child Health Services staff and completes these visits in a timely manner.
  • Communicates with parents and appropriate staff regarding health status of children with identified health concerns.
  • Personal, written, and/or telephone contact will be made and documented as to status, recommendation, and/or needs.
  • Visits Head Start families in the home as needed for consultation, information, and/or observation.
  • Obtains information from other health and education providers and incorporates their recommendations into health service plans for the Head Start child.
  • Provides initial and follow-up screenings (vision, hearing, height and weight) for Head Start children enrolled in a center base program. Works in conjunction with center staff to compete and document initial and follow-up screenings. Documents all action, completion and/or needs of the child.
  • Maintains cooperative working relationships with physicians, dentists and other local health care providers in order to provide optimal healthcare for enrolled children.
  • Refers parents/ child to appropriate community services for identified health needs and documents all such referrals.
  • Monitors whether Report of Referrals have been written to Nutrition Services for children who have nutritional concerns. Makes referral to the Nutrition Consultant as needed.
  • Arranges vouchers for examinations/follow-up treatment in conjunction with Family & Community Engagement Specialists’ efforts.
  • Participates in the Case Review process, as appropriate.
  • Administers first-aid or other emergency care as necessary when on site.
  • Reviews and updates child medical records for completion, follow-up and/or current compliance every 30 days.
  • Enters health data in Child Plus and reviews, updates, and codes the Health Information in Child Plus.
  • Conducts a monthly review with Family and Community Engagement Specialists to coordinate plans and communicate progress.
  • Maintains strict confidentiality at all times, including the ability to recognize situations in which confidentiality is required.
  • Attends all workshops and meetings as deemed necessary by the Program Nurse.
  • Attends all required staff and parent meetings and activities.
  • Other related duties as assigned.

Qualifications

Education:

LPN required, BSN or RN preferred.

Experience

  • Requires two years of experience in community health, pediatrics or school nursing.
  • Bilingual preferred: English/Spanish or English and other languages present in the local area.
  • Also required: Certification in First Aid and CPR within six months of the date assigned to this position.
  • Successful completion of a Level II background screening, local criminal record check and drug screening prior to hiring.

Knowledge, Skills and Abilities:

  • Knowledge of human development and health.
  • Knowledge of social services and health services available in the area.
  • Knowledge of computer data entry processes.
  • Knowledge of family-centered practices.
  • Ability to communicate ideas and instructions orally and in writing.
  • Ability to write reports and business correspondence.
  • Ability to speak before groups of individuals, such as Committees and Staff meetings.
  • Knowledge of human development and health.
  • Knowledge of social services and health services available in the area.
  • Knowledge of computer data entry processes.
