Mid-Level Provider FP/OB-Rexburg

Since 2007, Grand Peaks has been committed to providing a Medical Home to patients, where medical, dental, behavioral health services and clinical pharmacists work together and coordinate efforts in a team-based system of care. Grand Peaks consistently ranks highly in both employee and patient satisfaction. We are seeking a Certified Physician's Assistant or Family Nurse Practitioner with similar values to join our team of talented providers to help us better serve Eastern Idaho. The candidate will work within our Rexburg medical clinic, under the direction of the CMO, performing outpatient clinical tasks and functions.

Qualifications:

1. At least two years experience with family practice and OB patients preferred

2. Current Idaho State Medical License

3. Current D.E.A. Certificate

4. Board Eligible or Board Certified in Family Medicine. If Board Eligible at the time of hire, must be Board Certified within 3 years of hire date

5. Insurability (malpractice)

6. Current Cardiopulmonary Resuscitation Certificate (CPR)

7. ACLS or Advanced Life Support within 12 months of employment, if appropriate

8. Must be eligible to participate in CMS programs

9. Must pass a federal background check
