Job Summary and Qualifications

 

The Sepsis & Stroke Coordinator is a health care professional who works in collaboration with the medical and hospital staff to provide multidisciplinary care to the septic and stroke patient. The Coordinator provides clinical and professional expertise for patients, families and staff members. Scope of practice includes providing organized care to the patient with sepsis with focus on health promotion and disease prevention. The Coordinator also functions as an educator and coordinates care in critical care/sepsis patient care management/stroke patient management, ensuring optimal patient care management and outcomes. The Coordinator will collect and analyze date surrounding the sepsis and stroke patients to assist with documentation, coding, clinical care, and outcomes. Functions in a professional self-directed manner to assure that position responsibilities are met. Applies good verbal and non-verbal skills to achieve service goals of customer service and satisfaction. Collaborates with all members of the interdisciplinary team to achieve productive movement of the best practices within the organization. As requested, actively participates in accomplishing Facility, Division and Market Sepsis/Stroke service goals.

This position requires a thorough knowledge of nursing and principles adult education, policies and procedures, and regulatory requirements. The ability to assume responsibility and exercise authority is required. Interactive association and managerial tasks are inherent in this position. The ability to retrieve, communicate, and present data and information electronically, verbally and in written formats is essential. A professional demeanor and the ability to communicate effectively with patients, physicians, and staff are required. The ability to maintain confidentiality of information is essential.

 

  • Collects and analyzes data in order to identify current and expected needs of the stabile/acute/critical patients.
  • Instrumental in developing and prioritizing the plan of care to attain measurable goals
  • Implementation of the intervention identified and/or prescribed, and evaluates the results.
  • Assess and recognizes acute and chronic pain in all patients to assist in alleviating Suffering and improving quality of life.
  • Delivers medication based upon continual assessment, safety and evaluation of patient response.
  • Serves as a role model, consultant and clinical resources person to medical and nursing staff and other health care providers regarding their sepsis and stroke care.
  • Makes rounds on all patients within specific patient population to identify and manage patient population. Provide education and follow up of core measures.
  • Promotes efficiency and economy in the use of equipment, materials and supplies monitoring cost of resources associated with the care of sepsis and stroke patients.
  • Maintains open lines of communication with physicians, nursing, case managers and other healthcare professional regarding patient/family needs and identifies areas of improvement throughout the hospitalization.
  • Assists in the development of policies, procedures, standards and other resource materials pertaining to sepsis care. Works collaboratively with Directors/Managers of
  • Nursing/CNO to identify nursing competency assessment needs.
  • Assists in the development, implementation and assessment of quality improvement activities as it pertains to sepsis and stroke care.
  • Assists with formal and informal educational activities for nursing staff, based on assessed needs of the staff, new policies and procedures and quality improvement issues to improve patient care of sepsis and stroke patients.
  • Participates in patient care conferences
  • Participates in the development of individualized plan of care with the appropriate healthcare providers to progress the patient through the hospital process to the time of discharge.
  • Participates in the selection and preparation or educational materials for nursing staff and patient and/or families as it pertains to the care of specific patient populations.
  • Assumes responsibility for own professional growth as evidenced by involvement in professional nursing organizations and attendance at related seminars.
  • Establishes effective working relationships with staff, physicians, administrators and other departmental professionals on an ongoing basis.
  • Promotes and achieves application of sepsis and stroke clinical best practices towards effective and efficient patient care within the facility by proactively networking with Facility, Market, Division and Corporate and Sepsis/Stroke resources
  • Serves as chairperson for Sepsis and Stroke committee.
  • Educates medical staff, nursing staff, and ancillary staff on best practice for treatment Sepsis and Stroke patients.
  • Performs all other duties as assigned

 

What qualifications you will need:

 

  • Current FL RN License required or recognized Compact Licensure (subject to Florida State licensing requirements, including/not limited to ongoing eligibility and duration provisions)
  • Minimum of three years progressive experience.
  • Previous Meditech experience required.
  • Ability to establish and maintain collaborative and effective working relationships
  • Ability to communicate effectively in oral, written and electronic formats
  • Demonstrates analytical and critical think abilities with pro-active decision-making and negotiation skills
  • Ability to manage multiple tasks simultaneously
