KVC is seeking a Family Care Coordinator to work directly with families on a day-to-day basis while also managing and coordinating client care. Primary responsibilities include the following: establishing initial contact with families within 24 hours of referral to assess safety, initiate safety planning, and connect families with resources to meet immediate needs; conducting team meetings and maintaining communication with the family and providers to monitor progress toward treatment goals and eliminate barriers to service provision and goal attainment; facilitating the implementation of each family’s Action Plan by establishing contact with all providers identified on the plan, connecting providers with the family, maintaining ongoing communication with family and treatment providers, and coordinating care for the duration of services; making frequent and consistent contact with DCBS personnel; and providing direct services targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines. The Family Care Coordinator must maintain a flexible work schedule, including nights and weekends as necessary to meet families’ needs, be available to travel throughout the region, and participate in an on-call rotation. The Family Care Coordinator must have a Bachelor’s degree in Social Work (or other related field) with ability to provide in-home services to children and families and experience working with complex and multi-need families.

A valid driver's license and proof of auto insurance is required.
