Community Based Clinician - Serving the South Denver Metro area

Full Time

$70,000 to $82,000

*New Hire Sign on Bonus: $500 after 90 days; additional $1000 after 6 months

Job Summary and Purpose

  • Provides clinical case management, care coordination, and individual/family therapy for an evolving caseload of clients.
  • Works collaboratively from intake to discharge with internal and external multi-disciplinary team (MDT) members to ensure continuity and consistency of care for clients and families.
  • Provides clinical case direction and recommendations to both family support specialists as well as the MDT.
  • Actively participates in individual clinical supervision and other required meetings.
  • Maintains up-to-date documentation and overall quality assurance for assigned clients.
  • Requires a high level of independence due to satellite location.
  • May require travel to Tennyson Center for Children’s Denver location.

Essential Job Functions

  • Completes the intake family assessment within the required time frame including recommendations for services, level of care, and diagnosis.
  • Establishes criteria for client discharge from the time of intake.
  • Develops quality Service Plans based on the client’s diagnosis or reason for referral, which are community-based, family-focused, child-centered, and reflect measurable outcomes with a clear permanency plan.
  • Completes documentation of individual and family sessions, case management contacts, monthly progress reports, and other required forms of paperwork within required timeframes of Medicaid standards and other contracted referral sources. Expected billable hours per week is 24 hours.
  • Ensures the contracted services are being provided at the indicated frequency, length of stay is kept within indicated parameters, and the efforts of all treatment team members are congruent with the identified disposition plan for a given child.
  • Facilitates monthly staffings with the MDT to ensure treatment plan development and implementation. Attends other treatment-related meetings including county Human Services Team Decision-Making (TDM) meetings and/ or court hearings. Participates in supervision in person, by telephone, or by other multi-media avenues as needed.
  • Prepares and delivers case presentations to peers and other internal team members as required.
  • Provides in-home therapeutic treatment which utilizes a family systems approach that is strength-based and time-limited. Meets with families at flexible times throughout the week and on the assigned weekend day.

Other Job Functions

  • Facilitates transition of clients from TCC Denver residential program to the Community Based Services including traveling to and from the Denver location to facilitate this transition.
  • Attends scheduled internal trainings at Denver location and participates in continuing education to obtain or maintain licensure.

  • Provides crisis consultation and support to clients on caseload during regularly scheduled workdays. Develops safety plan with clients and plan for crisis on non-workdays. On days when team coverage is not available, on-call responsibilities will be assumed.
  • Assists with planning for ongoing services for all client’s post-discharge.
  • Facilitates coordination of care when other service providers are involved.
  • Maintains up-to-date individual professional license and/or certification. Ensures that a current and valid professional license or certification is always on file with the Human Resources Department.
  • Maintains the highest standard of cultural competency in all dealings with clients and staff.
  • Demonstrates knowledge and ability to implement agency policies, procedures, SPB’s, collateral policies, and COA’s guidelines.

  • Completes other related duties as assigned.

Physical Requirements

  • TCC requires staff in direct care of children to be able to perform physical restraints or

containments, as necessary. Clients are age range 5-18.

  • Assume a restraint for periods of time varying from 5-30 minutes.

Working Conditions

  • This position requires driving a company vehicle and visiting clients in their homes, in the community, and in schools.

Minimum Qualifications

  • Master’s Degree in social work, professional counseling, or marriage and family counseling required
  • Clinical licensure preferred LCSW/LPC/LMFT.
  • Must be registered on DORA.
  • Family therapy, residential or inpatient experience treating children and families highly preferred.
  • Bilingual English/Spanish Preferred.
  • Must have a valid Colorado driver’s license.

All positions require the successful completion of a criminal CBI background check, FBI fingerprint check, State of CO child abuse check (TRAILS), DMV driving record check, pre-employment drug test, pre-employment physical, academic credential verification, and reference check.
