Description
Good Shepherd Hospice is seeking a compassionate and experienced Patient Care Manager to join the Leadership Team in our INDEPENDENCE Office.  

Working under the supervision of the Executive Director, the Patient Care Manger is responsible for overseeing the clinical aspects of our hospice program, for ensuring the provision of quality care to patients and families, and for ongoing evaluation of staff and services. 

Our ideal candidate will possess strong leadership skills, a passion for excellent service, and the desire to support and mentor our Olathe hospice team. 
  • Full-time position / 8am to 5pm, Monday through Friday 
  • Minimum of 1 year of successful supervisory experience in hospice required.

Key Responsibilities
  • Provides day-to-day oversight and management of all clinical staff. 
  • Assures continuity of care for patients/families from admission to discharge or transfer to bereavement. 
  • Leads a weekly interdisciplinary team meeting focused on creating a positive team culture, reviewing patients, providing clinical education, and discussing operational needs. 
  • Establishes and maintains positive relationships with executives, managers, physicians, non-physician providers, support staff, and patients/families. 
  • Assumes the responsibility for the maintenance of patient records from intake through discharge or transfer to bereavement. 
  • Acts as a resource and mentor for staff for clinical issues, problem solving, documentation, and appropriate customer service behaviors. 
  • Assists with clinical recruitment, on boarding and training. 
  • Makes as-needed urgent visits to patients. 
  • Leads professional development and education efforts including ongoing supervising and training, mentoring/coaching, and implementing performance improvement plans as needed. 

Skills Knowledge and Expertise
  • Valid RN License
  • Basic understanding of the interpersonal dynamics of leading and developing effective working relationships within an interdisciplinary health care team. 
  • Must be of high integrity including maintenance of confidential information including all patient records 
  • Works as a team player, following directions and adhering to defined care plans 
  • Demonstrated commitment to the hospice philosophy of care
  • Can successfully pass background checks
