Care Coordinator (Scheduler).

As a Care Coordinator, you are responsible for ensuring that each caregiver and client receives exceptional customer service by coordinating & matching client and caregiver needs on a day-to-day/weekly basis.

Must have an upbeat, positive personality and possess excellent written and verbal communication skills. Must be able to work as part of a team and independently.

Duties include but are not limited to the following:

Schedule and coordinate day to day activities of caregivers and clients.

Build relationships with field staff, familiarize yourself with their preferences and abilities.

Review open shifts daily. Prioritize and schedule all open shifts.

Provide caregivers and clients with appropriate information regarding their schedules/changes.

Communicate continually with caregivers and clients to share pertinent information.

Respond promptly and courteously to all client/caregiver inquires.

Perform on-call coordinator duties as scheduled, which will include early mornings, late evenings & weekends.

Work with Executive Director & Care Coordinator Manager to assure proper care and staffing needs are being met.

Document all client and caregiver inquiries and alert Care Coordinator Manager/Executive Director as necessary to any urgent matters.

Review daily caregiver reports/notes and follow up accordingly.

May perform other duties as assigned
