The RN Director of Case Management provides overall leadership, direction, and strategic planning for Case Management services across the Health System network to promote and assure the advancement of Coordinated Care across the Care Continuum. The System Director of Case Management in coordination with system leadership, assures that Case Management and Care Coordination services effectively and efficiently meet the needs of all internal and external customers and are compliant with regulatory groups. The System Director of Case Management assumes overall responsibility, authority and accountability for the organization and operation of assigned departments. The RN System Director of Care Management has oversight over all aspects of daily operations to enable long-term and short-term goals to be achieved throughout the Care Continuum.

Qualifications:

• Registered Nurse required
• Bachelor's Degree in Nursing required
• Master's Degree required
• Director of Case Management experience in an Acute Hospital setting required
• Multi-Hospital campus leadership required
• CCM or ACM preferred

Responsibilities:

• Ensures quality of care & patient satisfaction through oversight of policies, procedures, and improvements
• Manages, develops, retains and inspires an engaged workforce
• Designs research, education & training to support cutting edge patient care and services
• Ensures operational profitability through effective budgeting and resource management
• Develops and executes long-term strategic business plans that support business growth
• Manages the physical & technological infrastructure and supports organizational structures movement initiatives
