Job Description

TeamPHCA is seeking a Direct Support Professional/Peer Specialist to join our team.

The ideal candidate must be able to support our clients and their property with dignity, patience, compassion, and respect. This person will encourage and remain empathetic to the clients at all times.

Compensation: $14-$18 per hour

Schedule: Schedule and start date will be based on the need of the individual and can change at any time. The individual has the right to choose his/her own schedule.

 

Responsibilities: 

Other duties may be assigned:

  • Home assistance – Provide light housekeeping, run errands or provide transportation if needed.  Accompany clients to appointments and assist with medications. Prepare meals, purchase food and provide personal assistance with general living needs.  Personal services could include toileting, bathing, feeding and grooming. Provide any financial aid if needed such as balancing checkbooks, going to the bank or helping the client to pay their bills and other community activities based on the individual's preferences and outcome action plan. Transport individuals in your personal vehicle to medical appointments, personal shopping, and recreation activities. Support individuals with identifying and facilitating volunteer opportunities or paid employment. Administer Medication, if applicable

 

  • Companionship – Provide companionship and conversation by stimulating, encouraging and assisting an individual and other community activities based on the individual's preferences and outcome action plan. Transport individuals in your personal vehicle to medical appointments, personal shopping, and recreation activities. Support individuals with identifying and facilitating volunteer opportunities or paid employment. Administer Medication, if applicable

 

  • Community Participation Support (100% in the community/Facility) – Provide support in the individual outside the community. This may include bowling, movies, volunteering, shopping, social clubs, special olympics. Transport individuals in your personal vehicle to medical appointments, personal shopping, and recreation activities. Support individuals with identifying and facilitating volunteer opportunities or paid employment. Administer Medication, if applicable

Requirements: 

  • High school diploma preferred
  • 3-6 months experience
  • A car that is currently registered
  • A valid Driver's License
  • Social Security Card
  • An operable cellphone
  • Must be able to complete a Criminal Background report.
  • Ability to lift up to 25 pounds at a time
  • Ability to reach, bend, kneel and stand for (sometimes) a long period of time 
  • ClearCare Preferred)

  • Developing Activity Calendars

  • EVV Training (Preferred)

  • Logging Same Day Progress Notes (Required)

  • Medication Log

  • ISP Implementation

  • Detection, Reporting, Prevention of Abuse, Suspected Abuse and Alleged Abuse

  • Individual Rights

  • Person-Centered Practices

  • Reporting Incidents

  • Seizure management: 1 year (Preferred)

  • human services: 1 year (Preferred)

  • Behavioral Health: 1 year (Preferred)

  • Defensive Driver Training Course (Preferred)
