As a clinician with PRN HH&T you will visit patients in SNFs, ALFs, and in their homes.

You are a therapist who:

· Holds or a current License/certification in Wisconsin

· Values building connections with their patients.

· Likes working independently while backed by a collaborative team.

· Has great written and oral communication

Job Duties Include:

· Evaluating and developing individualized patient treatment plans

· Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.

· Instruct Physical Therapist Assistants in treatment procedures to be utilized and provide supervision.

· Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan.
