Are you a Registered Nurse interested in working collaboratively with providers and clinical staff to support patients in an Occupational Medicine clinic? The Case Manager will facilitate all employee health, including communication with HR, employee and Supervisor, vaccine administration and compliance reporting. This Case Manager works in the ambulatory setting and is a member of the provider led interdisciplinary team. The Case Manager understands and adheres to established care management standards of care and coordinates care using evidence based clinical guidelines for surgical services.

What You'll Need:

  • R.N. (B.S.N. Preferred)
  • BLS certification required upon hire
  • 3 years R.N. experience
  • Care Management Experience preferred
  • Bi-lingual in Spanish preferred
  • Ability to work in fast-paced clinical environment is a plus

What You'll Do:

  • Respond to provider referrals and/or identify patients who need assistance with education regarding the services being provided and with navigating through the clinic.
  • Conduct initial patient assessments to establish understanding of their Workman's Compensation case and what is going to be done and what they can expect from the process.
  • Help facilitate communication between patients and their providers, and coordinate communication among all of patient's multiple providers.
  • Coordinate with care managers or patient navigators in other settings as appropriate.
  • Maintain electronic health records for employee health.
  • Operate within multiple electronic systems
