Principal Duties & Responsibilities:

  • Coordinate meetings with case managers, clinicians, community-based providers, and/or support workers.
  • Ask questions: learn who will be informed of any changes in care, resources, and/or treatment.
  • Recognize that caring for an individual who has challenging mental health issues can be demanding on the individual, families, supporters, and clinicians. Organize respite when needed.
  • Double-check all appointment times and places.
  • Ask for information and appointment details in writing – copies of forms, letters, or emails.
  • Find out information about mental disorder, and learn about the consumer’s journey, so that all are aware and can plan for the next stage.
  • Find out what organizations or agencies are involved in care and treatment.
  • Maintains a caseload established with the Service Director in order to work closely with programs in assigned service areas.
  • Assesses and evaluates adults to determine the appropriateness and need for case management services.
  • Assists in the person's entry into service and monitors the needed services throughout service system.
  • Provides coordination and comprehensive development of an Individual Treatment Plan (ITP) and monitors that plan as
  • required by established rules.
  • Follows State of Montana and A.W.A.R.E., Inc. rules, policies, procedures, and guidelines as they apply to this service.
  • Assures that any person serviced by Adult Case Management is screened for and assisted in enrollment in the SDMI
  • Program.
  • Must demonstrate – on a consistent basis – the willingness and ability to productively utilize work time in order to meet the needs of the individuals being served as directed, with the understanding that specific fiscal goals (billing, case load number maintenance, weekly) must be met on a monthly basis.
  • Records notes, billing units, assessments, referrals, and other data entry assignments via AWARE’s Electronic Healthcare Record System.

Required Skills, Abilities & Education:

  • Bachelor’s Degree in Human Services or a related field.
  • Minimum of one (1) year direct work experience.
  • Valid Montana Driver’s License.
