Company DescriptionHealthCare Support is actively seeking a Field RN Case Manager to fill an opening in Hillsboro, OR.
Daily Responsibilities for Field RN Case Manager:
- Ensure accurate and complete documentation, coding, and medication reconciliation in compliance with state RN practice laws.
- Collaborate with the interdisciplinary care team to identify and respond to patient changes in condition.
- Assist in various care coordination activities, including benefit determination, therapy coordination, family conferences, and managing members through acute medical situations or rehabilitation episodes.
- Shift: 8-5 M-F
- Associate degree or higher in Nursing (RN)
- Active and unrestricted license in Iowa
- Full COVID-19 vaccination and BLS certification
- Minimum 1 year working in a hospital or Long-Term Care facility as an RN
- Able to lift 30 lbs. and ability to perform physical assessment
- Ability to travel between facilities
Preferred Qualifications for Field RN Case Manager:
- 1+ year clinical experience, preferably in a long-term care setting.
- Experience addressing complex medical needs of patients with behavioral, social, and/or functional challenges.
- Proficient computer skills, effective communication, time management, and the ability to work in diverse care teams; access to reliable transportation.
