Hello, Oak Creek Rehabilitation in Kimberly, ID is actively looking to hire a full-time Assistant Director of Nursing/MDS Coordinator.

Job Description:

Assist the Director of Nursing with:

  • Active management of clinical systems
  • Team building, morale building, motivating the clinical team to provide the finest patient care
  • Assist with development/maintenance of nursing programs and activities
  • Complete medical forms, reports, evaluations, studies, charts, etc.
  • Any additional functions as directed by the DON;

MDS DUTIES:

  • Responsible to assign ICD-10 CM diagnosis codes to the residents' medical conditions
  • Accountable for case management of Medicare Part A and insurance case management
  • Expertly assess nursing home residents' needs, and coordinating personalized, resident-driven care based on those assessments
  • Oversee the patient assessment process, setting assessment schedules and assuring that assessments are done in an accurate and timely manner
  • Make Recommendations and transmit assessments to the Dept. of Health. Review the results to make policy and procedural recommendations to the management staff
  • Coordinate patient care plan in accordance with regulatory requirements. Ensure that appropriate facility resources are made available to patients and that patient care is delivered effectively
  • Create the schedule for all Medicare and Medicaid including:
  • Start Medicare coverage for newly qualified patients and/or send out denial letters
  • Keep updated on regulatory changes in Medicare coverage and help determine documents needed for Medicaid reimbursement.

Requirements:

  • Minimum associate degree. Bachelor's degree preferred
  • RN or LPN license in good standing
  • Minimum one year of successful nursing leadership experience in a long-term care facility preferred
  • Minimum two years' skilled nursing facility nursing experience required
  • Must show ability to read and write in a legible and understandable manner
  • Must speak and understand English
  • Extensive knowledge in MDS
  • One year of MDS experience in a skilled nursing/long-term care facility
  • Extraordinary attention-to-detail, capable of directing other people and managing schedules
  • Excellent communication skills, expert at working cooperatively as a member of team
  • Proficiency in MDS 3.0
  • Demonstrable working knowledge of relevant state and federal regulations
  • Resident Assessment Coordinator certification
  • Genuine love & concern for the geriatric population!
