Case Manager

The region’s leading network of healthcare providers, Inspira Health is seeking a *Case Manager *to join us at our Vineland, NJ location. You will play a critical role in facilitating the patient’s hospitalization, from pre-admission through discharge. *Full-time and per diem positions available.*



· Collaborating with physicians, nurses, social workers and other healthcare professionals to ensure the highest quality of patient care

· Performing initial and ongoing assessments to ensure effective transitions and prevent hospital readmission

· Ensuring the discharge plan is consistent with the patient’s clinical course, continuing care needs and covered services

· Providing referrals to social work services to patients and/or families that require further assistance



*Qualifications: *

· BSN required

· Licensed Registered Nurse in New Jersey required; Case Management Certification preferred

· 3+ years of experience in a hospital setting; 1+ years of case management experience preferred

· Experience in assigned unit or service line with previous utilization management

· Previous experience with regulatory and compliance requirements preferred

· Strong communication, interpersonal and administrative skills
