Job Title: House Coordinator

Reports to: Designated Coordinator/Management

Department: CRS

Job Summary:

 

As a House Coordinator within our 245D company, you will assume a vital role in managing the daily operations and ensuring the well-being of individuals in our residential programs. Your responsibilities include overseeing staff, coordinating services, and maintaining a positive living environment in accordance with 245D regulations. From staff scheduling to addressing individual needs, your attention to detail and commitment to compliance will contribute to the high standards of care we uphold. Collaborating with internal teams, you will play a key role in fostering a supportive and inclusive atmosphere for individuals served. If you are dedicated to providing quality care, have strong organizational skills, and understand the importance of regulatory adherence, we invite you to join our team as a House Coordinator within the 245D framework.

Duties and responsibilities:

 
  • Oversee the management of a one or more individuals served caseloads
  • Assist individuals served with management and scheduling of their appointments and related transportation needs
  • Schedule direct support staff to ensure coverage of all assigned hours
  • Train and supervise direct support staff according to recipient needs and goals
  • Maintain compliance with federal and state standards and laws
  • Assist with the coordination of meetings between staff, individuals, case managers, and guardians
  • Gather current and accurate information in order to support individual needs and goals
  • Assist with the completion or creation of 245D-required paperwork that is unique to the individual
  • Maintain an average of 85% (6 hours) client contact time and 15% (34 hours) administrative time weekly
  • Communicate frequently with guardians, management, and caseworkers
  • Attend, assist, and engage in team meetings as needed
  • Ability and willingness to fill in Behavior Technician roles as needed
  • Maintain and complete continuing education as required
  • Reporting suspected or known abuse according to company policy
  • Uphold confidentiality and privacy of individuals served and staff members
  • Other duties as assigned

Required Skills/Abilities:

  • Clear background check with the MN Department of Human Services
  • Knowledge of federal and state healthcare laws and regulations
  • Ability to track and chart data on a daily basis in compliance with company expectations
  • Ability to understand and implement a variety of Community Support Plans and Individual Service Plans per individuals outcomes and goals
  • Willingness to travel to and from surrounding area communities as needed
  • Vehicle, driver’s license, and valid auto insurance per the State of Minnesota
  • Dynamic communication skills, working techniques, and ability to multitask
  • General computer skills/knowledge
  • Working phone with internet access
  • Monthly trainings as assigned

Qualifications:

 
  • Must be at least 18 years of age and possess a High School Diploma, GED, or equivalent.
  • Experience working with complex medical or behaviors individuals
  • Associate’s degree, or higher, in social work, human services, or similar studies
  • 2+ years of experience working with individuals of varying needs and diagnoses
  • Current CPR and First Aid Certification is preferred
  • Multilingual - fluent, ability to translate if necessary
  • 1+ year of training and supervisory experience
  • 245D familiarity
  • Clear driving record (No DUI/DWI convictions in the past 5 years)
