Responsible for providing Assisted Living/Memory Care/Personal Care Home care services to residents in accordance with existing licensure, established standards and practices and with the guidance and supervision of the Resident Care Director.

$27/hour

QUALIFICATIONS AND REQUIREMENTS:

· Graduated from an accredited program for a Licensed Vocational or Practical Nurse (LVN/LPN) and is currently licensed in the State

· Current CPR certification required

· 2-4 years of nursing experience in a health care facility

· Experience in geriatric nursing, emergency care, first-aid

· Must obtain valid Continuing Education credits as required by the State

· Desirable to have training in the special needs of the cognitively impaired resident i.e., Alzheimer, and Dementia training

· Must be able to cope with the mental and emotional stress of the position

· Must possess the ability to work effectively with physicians, management, employees, and other outside contacts

· Possess the ability to make independent decisions when circumstances warrant such action

ESSENTIAL FUNCTIONS:

· Ability to perform physical assessment of resident and identifies problems related to physiological, psychological, environmental, and social factors

· Administer medication per physician orders. Observe for any adverse reaction and notify supervisor and/or physician accordingly

· Meet with Resident Care Director on a regular basis to communicate plans, highlights, special challenges, required support, etc.

· Ensure quality and safe delivery of health services to residents

· Collect and analyze pertinent health related information

· Obtain physician’s order prior to initiating treatments or changes in treatments

· Perform administrative requirements, such as completing necessary forms, charts, reports, etc., and submit these as may be required

· Assesses Life History and Preferences for each resident; maintains Communication Book; takes report from staff and refers clinical issues to the Resident Care Director for assessment and planning

· Observe and report changes in resident’s functional status and report to Resident Care Director and Care Staff

· Assess and update resident interests, needs and strengths and help set service plans accordingly

· Implement plan of care formulated by physician and Resident Care Director

· Utilize safe and appropriate techniques/nursing skills in providing resident care

· Report pertinent changes in patient’s condition to the Resident Care Director and/or physician

· Report changes in patient status to the office such as hospitalization, discharges, and physician appointments

· Provide accurate and timely documentation consistent with plan of care

· Assess and provides resident and family education and information pertinent to diagnosis of plan of care

· Communicate physician’s feedback with patient and/or family

· Follow established guidelines concerning the storage of drugs

· Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the community, pharmacists, and physicians

· Follow infection control and universal precautions when providing patient care

· Communicate directly with other disciplines involved in resident care on a regular basis, including weekends

· Participate in team meetings to focus on resident problems

· Maintain safe and healthy work environment and use safe work methods and procedures in accordance with company clinical standards

· Assist resident in learning appropriate self-care activities

· Organize and monitor human and material resources necessary for meeting needs of resident

· Recognize emergencies and life-threatening situations and initiate appropriate actions as established by the agency

· Document all information in a timely and accurate manner

· Follow all appropriate safety rules, guidelines, and precautions

· Maintains confidentiality and resident rights, regarding all resident personal information

· Contact office with any changes in visit schedules

· Participate in staff development activities

· Cooperates with co-workers including assisting with orientation

· Follow dress code demonstrating appropriate appearance and personal hygiene

· Other duties as assigned

REQUIRED COMPETENCIES

· Maintain confidentiality of verbal and written information pertaining to residents, employees and community operations

· Adaptability- able to change tasks quickly as business needs dictate

· Use of computer for electronic medical records, including EMAR

· Strong attention to detail

· Ability and desire to work as a team member

· Neat appearance, adherence to dress code and good personal hygiene is expected

WORK ENVIRONMENT AND PHYSICAL DEMANDS/TRAVEL REQUIREMENTS

· Lifting, pushing, or pulling up to 50 pounds, using proper body mechanics

· While performing the duties of this job, the employee is regularly required to stand, bend, talk and hear. The employee is frequently required to walk. The employee is frequently required to use hand and finger to touch, handle and feel and required to reach with hands and arms. Must be able to meet certain physical demands of standing, bending, lifting, stooping or performing other work requiring physical exertion such as lifting (up to 50 lbs). Must be able to climb flights of stairs

· Must be able to sit, stand and walk for extended periods of time

· Must be able to endure the resident’s environment, which may be unsanitary and uncomfortable

· Must be able to assist residents to roll in bed, sit up from a lying position, stand from a sitting position, transfer assist to and from w/c and provide stand-by assist during resident ambulation or other activities of daily living

· Possible exposure to unpleasant odors

· Possible exposure to chemicals as identified in the MSDS Manual

· Possible exposure to verbal aggression from vendors, guests, residents and/or family members

· Continuous exposure to clients and/or family members who may be under stress

· Employee may be subjected to hostile and emotionally upset residents, family members, personnel, visitors, etc
