Licensed Practical Nurse (LPN) - Home Care

Job Title: Licensed Practical Nurse - LPN Visit Nurse
Company: Home Health Care, Inc.
Schedule: Full-Time or Part-Time Availability

Compensation: $32 - $33 per visit

Licensed Practical Nurse - LPN Visit Nurse Overview:

• Provide patient cares according to client’s care plan and needs
  
• Perform wound care treatments and and assist clients with medication management
  
• Teach the client and/or family members about cares and support skills
  
• Communicate regularly with RN Case Managers, Supervisors, and the Care Team
  

Licensed Practical Nurse - LPN Visit Nurse Qualifications:

• Licensed as a Practical Nurse / LPN in the state of Minnesota
  
• Driving is required; must have a reliable vehicle with current insurance and a valid Drivers License
  
• Demonstrated ability to work autonomously
  
• Prior Nursing experience in a TCU, Hospice, or Home Health setting
  
• Experience with Electronic Medical Records (EMR) software is preferred
