This is split position which will work part-time with our PATH program and part-time with our Housing specialists.

Duties & Responsibilities:

CM II-PATH:

  • Provide advocacy, linkage, and referral services on behalf of chronically mentally ill.
  • Provide intake, screening, referral and case management services.
  • Assist in maintaining an active liaison with community agencies serving the "high risk" population with modified PACT program.
  • Coordinate and manage indigent medication/patient assistance program.
  • All other duties as assigned

CMII-Housing Specialist:

  • Will serve as the clinic’s Housing Specialist and will assist persons and other staff to access housing services through Community Cares Partners COVID 19 Relief, local continuum of cares as well as other housing resources.
  • Educate staff on the Community Cares Partners COVID 19 Relief program and how to access services.
  • Will conduct outreach and engagement to local communities through enrollment fairs and other opportunities to connect people and agencies with Community Cares Partners COVID 19 Relief.
  • Will provide housing counseling, housing-related services for survivors of domestic abuse or human trafficking, fair housing counseling, housing navigation for ERA programs or to find housing, and mediation between landlords and tenants.
  • Establish relationships with landlords in the area to create housing opportunities for persons enrolled in CREOKS services.
  • Create housing resource lists and help apply for grants or other housing support opportunities.
  • Facilitate and teach the philosophy to staff and program participants that recovery is possible for everyone.
  • Support program participants in completing a Wellness Recovery Action Plan or Wellness Self-Management Plan as well as a safety plan as needed.
  • Provide education on health-related issues consistent with their training and experience.
  • Educate and assist program participants with rights, responsibilities, and appeals.
  • Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS rules and regulations.
  • Advocate on behalf of consumers.
  • To read and be knowledgeable of all current CREOKS policies and procedures.
  • Maintain productivity averages of direct service to clientele as set by CREOKS.
  • Coordinate and manage indigent medication/patient assistance program as assigned by the Site Director.
  • Turn in billing and associated documentation as mandated by CREOKS policy.
  • Attend weekly staff/treatment team meetings to support the community tenure protocol.
  • Responsible for participating in mandatory training such as nonviolent crisis de-escalation and etc.
  • Provide services and supports to all program participants within the community (to include a participant’s residence, etc.) consistent with the Care Manager’s education, experience, training, and scope of practice.
  • All other duties as assigned

Qualifications:

  • Minimum of Bachelor's degree in a mental health related field or a Bachelor's degree and at least 2 years supervised experienced in a treatment setting.
  • Maintain current case management certification.
  • Must be in recovery for at least one (1) year from mental illness and/or co-occurring disorders.
  • Ability to communicate needs to others.
  • Ability to work with the public, landlords, and other agencies in order to establish productive collaborative relationships and promote CCP.
  • Good verbal and written skills.
  • Ability to share a story of recovery to individuals and groups.
  • Knowledgeable of mental illnesses and co-occurring disorder.
  • Successfully complete the ODMHSAS CM2 Training and Test to be credentialed
  • Must complete the orientation training with Community Cares Partners.
  • Must complete the “Housing+ Endorsement” training through ODMHSAS at the first available opportunity.
  • Become trained and certified as a Wellness Coach according to ODMHSAS standards.
  • Successfully complete the Tobacco Free Support Group Training.
