The hours are Monday-Friday 8:00 a.m.-4:00 p.m. No weekends! 
Starting at $15.00 Hr.

ROLE OF THE LIFE SKILLS PRE-VOCATIONAL INSTRUCTOR/COORDINATOR

The Life Skills Instructor is responsible for assisting Individuals with intellectual and developmental disabilities (I/DD) achieve their highest level of independence. The Life Skills l Instructor focuses on providing support to individuals seeking employment, using a person-centered approach. It is the expectation that the Life Skills Instructor provides support and guidance in accordance with the scope of services to ensure the best outcomes towards outlined goals.

ESSENTIAL FUNCTIONS

  1.  Service Delivery
  • Monitor the health and safety of all individuals.
  • Assist Individuals with creating and documenting specific employment related Implementation Plan Goals.
  • Monitor Individuals progress toward IP goals.
  • Develop learning and work-related experiences where the individual can develop general, non-job-task specific skills that lead to competitive employment.
  • Present meaningful activities to aid the Individual toward their goal of employment.
  • Coordinate, schedule speakers and other community leaders to enhance work-related skills and provide real life work scenarios.
  • Plan and implement activities that align with each Individuals’ program goals, enrichment, and physical activity utilizing curriculum and materials provided by the program.
  •  Present and monitor daily classes to ensure are inclusive, engaging, and interactive.
  • Ensure familiarity with Individuals’ behavior plans.
  • Coordinate community outings.
  • For Individuals with a behavior plan, ensure understanding of protocols, communicate protocols to appropriate instructors.
  • Document behavior plan information as required.
  • Establish and maintain a supportive and professional relationship with individuals, their families, and circle of support.
  • Provide individual-specific service in compliance with service descriptions as defined in service directory/specific attachments and core assurances.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout the program.
  • Recognize and note physical and/or emotional changes in an individual’s condition and seek appropriate intervention. Report all changes to program manager or director within 1 business day.
  • Assist individuals with meals, personal hygiene, transitioning or other tasks, when needed.
  • Assist with daily arrival/ dismissal duties and tasks.
  • Contact the program director to report any difficulties, accidents, incidents or concerns immediately.
  • Complete all assigned documentation requirements. Including notes and progress summaries towards support plan/ implementation plan goals 
  • Enter complete notes daily into CRN selected system.
  • Prepare General Event Reports using CRN documentation system and submit to required parties based on APD guidelines.
  1. Compliance

Ensure documentation and submission of all incident reports in accordance with the APD guidelines.

Communicate all incidents to the program manager.

  • Maintain confidential information and ensure compliance with HIPAA.
  • Attend all trainings as directed and provided by the employer as required of Med-Waiver providers.
  1. Other Duties
  • Aid and assist individuals onboarding and offloading from transportation.
  • Communicate building maintenance issues and assist in coordination of resolution.
  • Serve as a positive role model for adults in the program through appropriate dress, speech, attitude, and courtesy.
  • Ensure department or classroom area is cleaned and sanitized per CRN safety statement and standards.
  • Comply with company policies and procedures.
  • Other projects and duties as assigned.

SUPERVISORY RESPONSIBILITY

  • Supervision of all Individuals receiving Pre-Vocational Service
  • QUALIFICATIONS
  • High School diploma or equivalent, 
  • One year of experience in a medical, psychiatric, nursing, or child/adult care setting or direct care with individuals with intellectual and/or developmental disabilities.
  • One year in an instructional role, teaching, demonstrating, etc. preferably working with Individuals with Intellectual and Developmental Disabilities
  • Valid state driver’s license.
  • Must pass Level II Background screening.
  • Must pass drug screening.
