Job Summary:
Bloom is seeking an exceptional Behavioral Health Care Consultant to join the team! This position will be joining Bloom’s Behavioral Health team, reporting to the Director of Specialty Services, with a focus on caring for Bloom’s geriatric patients with mental health challenges and/or dementia as well as young disabled patients with psychiatric needs.

Responsibilities:
  • Mental health assessments, including administration of validated scales e.g. PHQ-9, GAD-7
  • Provide lifestyle coaching to address issues such as anxiety, depression, sleep disturbances, and loss of function
  • Use multiple modalities to engage with clients like motivational interviewing, problem solving therapy, behavioral activation, and provide grief support
  • Coach caregivers and family members to support and advocate for patients, especially for vulnerable patients with issues such as dementia
  • Monitor symptom severity, treatment adherence and side effects
  • Track client progress through the use of a registry
  • Report results to primary medical provider and/or the consulting psychiatric consultant
  • Identify social service needs and refer to Bloom’s medical social work where appropriate
  • Participation in weekly caseload review with psychiatric consultant
  • Ongoing collaboration and coordination with treating providers
  • Relapse prevention planning and preparation for discharge from active treatment
  • Care management to be delivered in client’s homes or assisted living communities, but some virtual visits may be warranted
Qualifications:
  • Education: A bachelor's degree in psychology, counseling, social work, nursing, or a related field required. Master's degree or higher in clinical psychology, counseling psychology, social work, or a related field.
  • Licensure: LCSW/LSW, LPC, or PSY
  • Clinical skills: Behavioral health consultants should have strong clinical skills, including the ability to assess mental health needs, develop treatment plans, and provide evidence-based interventions. Knowledgeable about a range of therapeutic approaches, such as cognitive-behavioral therapy, dialectical behavior therapy, motivational interviewing, and solution-focused therapy.
  • Communication skills: Excellent communication skills are essential for building rapport with clients, collaborating with other members of the healthcare team, and providing education and support to patients and their families.
  • Cultural competence: Behavioral health consultants should have an understanding of and respect for cultural differences and how they may impact mental health and healthcare seeking behavior. Cultural competence is important for providing effective, culturally sensitive care to a diverse range of patients.
  • Problem-solving skills: Behavioral health consultants should be skilled at problem-solving and be able to help clients identify and address challenges in their lives. They should also be able to adapt their approach to meet the unique needs and preferences of each patient.
  • Empathy and compassion: A strong sense of empathy and compassion is essential for working effectively with individuals who may be experiencing mental health challenges. Behavioral health consultants should be able to empathize with their patients' experiences and provide support and encouragement in a nonjudgmental manner.
  • Ethics and professionalism: Behavioral health consultants must adhere to ethical guidelines and professional standards of conduct in their practice. This includes maintaining patient confidentiality, respecting boundaries, and seeking supervision or consultation when needed.
  • Continuing education: Given the evolving nature of the field of behavioral health, ongoing professional development and continuing education are important for staying current with best practices, emerging research, and advancements in treatment modalities.

The pay range for this role in the state of Colorado typically falls between $90,000-$105,000 annually with the potential for performance-based bonuses and other benefits.
