Requirements

• Provides support for activities of daily living which include:
• Homemaker
• Personal care services for the client in his/her home as identified
• Taught and monitored by the PHC Plus based on the individual client needs
• Client needing full assistance with all ADL's - dependent on Caregiver for all personal care

Shifts are Friday/Saturday 7:30 AM to 10:00 AM; 12:00 PM to 3:00 PM; 6:15 PM to 8:40 PM

Monday and  Wednesday: 6:15 PM to 8:40 PM
