RN Director of Case Management
Inland Empire, California

The RN Director of Case Management has overall responsibility for managing and coordinating department activities. The RN Director of Case Management ensures staff compliance with organizational policies and external regulatory agencies. The RN Director of Case Management coordinates the integration of the department's patient care and discharge planning processes with related hospital departments and external agencies to ensure continuity of care and optimal clinical resource utilization. The RN Director of Case Management provides oversight for all aspects of daily hospital Case Management operations and is accountable for achieving established outcomes through actively engaging interdisciplinary teams and external stakeholders. The Director of Case Management directs and oversees the day-to-day functions of Case Management services under licensed inpatient and outpatient areas.

Qualifications:

  • Registered Nurse required
  • Bachelor's Degree in Nursing required
  • Master's Degree required
  • Director of Case Management experience in an acute hospital setting
  • Certified Case Management (CCM) preferred

Responsibilities:

  • Directs and evaluates departmental operations, including the Case Management model, staffing, use of information technologies, onboarding, and staff competencies to achieve performance and quality objectives
  • Allocates resources to effectively staff department and meet productivity and quality goals
  • Provides oversight of Case Management core functions and practice
  • Ensures the adherence to Care Coordination and discharge planning processes, ensuring timeliness, quality, and proper documentation
  • Works with the Facility CFO and Division Case Management Leader to achieve established goals and expectations
  • Assesses and improves the department's performance by evaluating operational processes
  • Monitors performance through analyzing data, and implementing sustainable performance improvement activities
  • Ensures compliance with policies and SOP
  • Develops, implements, and maintains a comprehensive and fiscally sound Case Management program
  • Orchestrates cohesive patient-centered care practices which address the needs of the patients from admission through discharge and across the Continuum of Care
  • Establishes and maintains processes to oversee Case Management onsite reviewers from payer insurances/organizations
  • Maintains a positive working relationship with payers and keeps each payer informed of changes during in-patient stay, discharge plan, and resources needed for safe and timely discharge
  • Substantial knowledge of Case Management practice, state and federal healthcare regulations, finance, performance improvement, quality, patient safety, evidenced-based clinical practice standards, medical staff structure and legal aspects of care
