Job Description
Schedule: 9:00am-5:30pm. 4 days on, 2 days off rotating schedule. Must be willing to work scheduled weekends and holidays. This position is in-person only.

The Case Manager will carry an individual case load for clients with primary Mental Health and /or co-occurring disorders. Those interested in the position MUST have experience dealing with acute clients that are often in psychosis, have mood disorders, and in some cases have severe SI.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned
Develop person-centered care plans, including prioritized problem lists, SNAP, client-reported goals, measurable and achievable objectives, and interventions. Identify and prioritize individual needs and problems.
Provide group therapy sessions utilizing Evidence Based Practices.
Document client progress toward treatment plan goals and objectives in individual progress notes.
Develop solid transition and discharge plans and summaries for assigned caseload.
Prepare clinical documentation accurately and within agency time frames.
Intervene during crisis intervention situations..
Participate in the agency’s Quality Management system, Orientation, staff meetings, and in-service training.
Provide accurate assessment and referral when appropriate.
Provide appropriate treatment interventions relative to the specific needs of each client.
Adhere to code of ethics, legal aspects of clinical practice, professional standards, duty to warn, abuse and neglect reporting policies and procedures, and exercise professionalism at all times.
QUALIFICATIONS, EDUCATION and/or EXPERIENCE

Education Requirements: Minimum of a Bachelor's degree in a relevant field of study.
Successful completion of Criminal Background Checks through DBHDS, Background Investigation Unit, and VDSS Registry Searches, in addition to passing the ADP Selective Screening Services background check
Possess valid driver’s license
CPR/First Aid Certification
Current T.B. clearance
Physical Requirements: Ability to lift 20 lbs.
