Overview

The Lead Outpatient Pharmacist provides daily oversight of the provision of quality pharmaceutical care services at the pharmacy department. S/he has the responsibility for overseeing and directing the pharmacy team and addressing any workflow issues. The Lead Outpatient Pharmacist will also play a liaison role with the medical staff and other hospital staff members. S/he will participate in dispensing activities, medication delivery, drug utilization review, adverse drug reaction monitoring, and will provide medication information and counseling to the patient. The Lead Outpatient Pharmacist supports the operational manager by guiding support staff, serving as a role model, and actively participating in assigned tasks and projects.

This position's work location is on site only.


Responsibilities

Patient Care
*Performs pharmacy clinical services to optimize drug therapy and documents clinical interventions.

*Provides accurate, clinically relevant drug information to medical providers, nurses and other allied health professionals.

*Prepares specialty compound prescriptions as needed.

*Delivers medications to the patient bedside prior to discharge, provides medication counseling and responds to medication related questions as needed.

*Identifies, prevents, and investigates potential and real adverse reactions as well as medication errors.

*Provides medication counseling to patients at the pharmacy level and responds to any medication related questions.

*Review and interprets medical providers orders properly, screens for incompatibilities, drug interactions, medication duplicates or contraindications.

*Responsible for contacting prescriber, nursing personnel or inpatient clinical pharmacist regarding any question on drug orders.

*Responsible for the accurate dispensing and labeling of medication orders. *Competent using various pharmacy technology products such as pharmacy software system, Point of Sale, electronic health records, automated dispensing machines, clinical decision support/data mining tools.


Administrative
* Collaborates with the pharmacy manager and other pharmacy team members to achieve compliance of department policies and procedures.

* Represents the pharmacy manager (when not present) during State Board inspections or Accreditation inspections.

* Supervises, train and educates new hires as well as other staff members as it relates to the department workflow, policies, procedures, and other areas/needs as needed.

* Supports with the training and teaching of pharmacy students.

* Performs all operational aspects of retail pharmacy including but not limited to computer operations and billing.

* Assists in maintaining medication inventory and medication supply from vendors.

* Exercises applicable legal and professional controls on controlled substances.

* Reports adverse drug reactions, medicaton errors and good catches on MIDAS or any other system.

* Serves as a resource to staff in caring for complex patients.

* Supports staff with end of day closing duties.

* Maintains knowledge and guides other staff members during downtime procedures.


Leadership
* Directs and re-directs the pharmacy workflow as needed.

* Provides oversight and the development of the team concept within the pharmacy department.

* Serves as a positive role model for department staff and actively participates in assigned special projects.

* Collaborates with pharmacy manager and staff on all aspects of safety and quality improvement.


Compliance/Safety
* Stays current and complies with state and federal regulations/statutes and company policies that impact the employees area of responsibility.

* If required for position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.

* Completes all company mandatory modules and required job specific training in the specified time frame.

* Responsible for reporting any safety related incident in a timely fashion through the Midas/RDE tool; attends all safety related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.


Qualifications

Education
Bachelor of Science in Pharmacy from an accredited college of pharmacy - Required

Doctor of Pharmacy degree from an accredited college of pharmacy - Preferred


Certification & Licensures
Arizona State Board of Pharmacy license that is active and in good standing - Required at date of hire


Experience
Minimum of (1) year of experience in a retail or alternate care pharmacy- Required


Previous pharmacy supervisory/management experience - Preferred
