Summary

Provides nursing care and coordinates medical/psychiatric care for clients; provides clients with continuity of care. Responsible for the provision of nursing services for the program.
If working at PACT: Works collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care program for all consumers. Is an integral member of the PACT team who works to ensure safety, best practice and high quality standards of care are maintained across the program. Coordinates a wide range of self-management support and disease registry activities for the entire consumer population. Conducts physical health assessments and maintains consumer medication regimen according to Psychiatrist orders.

Essential Functions and Responsibilities

  • Assures staff coverage and arranges scheduling changes when required
  • Monitors ongoing program development by assessing clients’ needs and referral sources, evaluates program effectiveness and reviews statistical data, caseloads, and staff performance per project evaluation model
  • Keeps apprised of program compliance regarding quality assurance and clinical records; monitors completion of required paperwork and forms, and reviews progress notes and Medicare compliance
  • Reviews and monitors client’s progress through supervision and disposition of cases; confers with other programs and agencies, doctors, etc., as required regarding client welfare
  • Administers medication and maintains clinical charts and records; observes, records, and initiates appropriate action concerning client reaction to medication and therapeutic procedures
  • Assists in assessment of client’s mental status on a daily basis
  • Monitors activities and job duties of staff assigned to their shifts
  • Performs a variety of professional medical nursing duties under the direction and supervision of the Psychiatrist; collects routine laboratory specimen, administrates injectable medications and emergency medication, communicates with pharmacies regarding doctor’s orders for medications and related duties
  • Maintains inventory of routine and/or locked medications in the medication storage. Assists Psychiatrist in maintaining stock of as needed, locked medications by ordering through pharmacy and assures correct count continually
  • Documents care of consumers according to nursing standards of care, agency policies and procedures
  • Performs nursing assessments in order to identify clients’ needs and plan of care. Provides treatment for existing and emergency health care conditions. Assesses for medical appropriateness for admission and continually assesses throughout the clients’ stay
  • Consults with Psychiatrist if change in medical status occurs, which may or may not include a referral to a more appropriate level of care
  • Attends staff meetings, workshops, and seminars to learn agency policy, rules, regulations, procedures; participates in ongoing in-service training as well as pertinent external training
  • Maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment
  • Assists in the development and implementation of programmatic goals and objectives
  • Supports and assists with the facilitation of Care Plans for consumers within the Red Rock Health Team
  • Performs other duties as assigned

Qualifications

  • Registered Nurse in the state of Oklahoma
  • Knowledge of psychiatric nursing, practices, techniques, and procedures
  • Knowledge and experience working with administration of psychotropic medications
  • Demonstrated skill in planning and directing the work of professional staff; in dealing effectively with people; in evaluating situations and choosing an appropriate course of action; and in communicating ideas clearly and concisely, both verbally and in writing
  • Must be able to work flexible hours as required
  • Commitment to the mission of Red Rock BHS
  • Three years of experience in nursing or related field preferred
  • Experience in psychiatric nursing preferred
