Job Objective: The objective of this position to provide strength-based case management, supportive services, assessments, and crisis intervention to homeless families to promote self-sufficiency in order to obtain and maintain housing.

Essential Functions:

  • Conducts intake assessments which include: required documentation for admission, referral needs, explanation of program and expectations
  • Complete written assessment for all new families within 72 hours of admission into program and maintain confidential client case histories, requests, findings, and services rendered
  • Schedule and develop a case plan with the head of household/spouse/partner within one week of intake which will include short and long term goals with objectives; update case plan as needed
  • Schedule regular meetings with participants to review and update goals and objectives
  • Provide information and referral services and advocacy services i.e. court appointments, landlord/tenant conflict resolution, etc. as needed
  • Develop a budget plan with the household and assist with financial requests in accordance with established schedules and guidelines
  • Coordinate case management efforts with all staff and contracted services to meet family and children's needs
  • Develop a discharge plan with participants which addresses permanent housing, economic and emotional stability
  • Enter required information regarding program participants into the HMIS program designated by the Continuum of Care in a timely manner
  • Facilitate educational support groups for adult residents
  • Assist with answering the phone and taking messages, receipt and inventory of donations, and assist with keeping the shelter director informed of needed maintenance and supplies
  • Actively promote community relations by representing the mission of The Salvation Army, serve as a liaison with area schools, attend monthly metro case managers and Homeless Coalition meetings
  • Meet at least weekly with the shelter director and attend in-service trainings
  • In the absence of the Shelter Director supervise staff and volunteers, schedule staff, compile records, prepare reports, and submit statistical information as requested
  • Assist with the development and implementation of policy and procedures
  • Rotate on-call schedule with the shelter director

Minimum Qualifications:

Education: Bachelor's Degree in Social Work or related field required; relevant experience, as determined by The Salvation Army, may be considered in lieu of education.

Experience: A minimum of three years of case management experience with low income, minority populations, and/or those with substance abuse, mental health, and homelessness issues

Certifications/Licenses: Licensed Bachelor of Social Work or certification in Case Management preferred

Skills/Abilities:

  • Computer skills in Microsoft Word and Excel required, ability to effectively communicate orally and in writing
  • Must have the ability to work independently and with other staff members
  • Demonstrate the ability to be non-judgmental in approach, allowing clients their right to self-determination, and treating all with dignity and respect
  • Must have an interest and ability to work with people and program participants of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner
  • Ability to solve complex problems, make appropriate judgments and decisions
  • Skilled in conflict resolution and trauma informed care
  • Ability to speak and understand English in a manner that is sufficient for effective communication with supervisors, employees, and program participants

Supervisory Responsibility:

This position has the authority to assign and check work of the client advocates. This position may recommend employment status changes to the shelter director, and assist with making disciplinary decisions but will not formally evaluate performance of other employees.

Physical Requirements: Include sitting, walking, standing, stretching/reaching, and lifting without assistance a minimum of 10-25 lbs. Reasonable accommodation may be made to enable individuals to perform the essential functions of this job.

Travel: Travel will consist of attending Social Service Conferences (which are both local and out of the area) at least once a year, and other monthly meetings and trainings to best meet the educational requirements of the position.

Driving: This position does require driving to assist with obtaining program needs. Therefore, must possess a valid driver's license from the state of residence and must be approved through The Salvation Army Fleet Safety Program to drive either a Salvation Army or personal vehicle on Salvation Army business.

Working Conditions: Work is performed in a 24/7 residential living environment. This is a full-time non-exempt position and will require some weekend and evening work.
