Position Summary:

This position will have responsibility for providing direct hospital diversion, jail diversion, and acute crisis services for Catholic Charities. The Crisis Diversion Outreach Case Manager will have primary responsibility for face-to-face mental health diversion screenings, interventions, transitional case management, and assessments in addition to specific law enforcement second- responder duties.

Provides outreach services to individuals experiencing acute emotional crises, with the goal to diverting the individual from incarceration or hospitalization into the least restrictive setting possible and conducts assessments across systems to determine eligibility for diversion into safe and appropriate treatment options. When appropriate, coordinates requests for evaluation under the Involuntary Treatment Act and appropriately utilizes crisis intervention techniques for individual and community stabilization.

Responsibilities:

Law Enforcement / Jail Diversion

  • Acts as Liaison between law enforcement agencies, Jail Administrator and Mental Health Professional, and City/County Prosecutors regarding recommendations for jail diversion services for an identified individual.
  • Liaison shall occur between Wenatchee Police Department, East Wenatchee Police Department, Chelan County Sheriff, and Douglas County Sheriff.

Community Liaison

  • Act as liaison to behavioral health and other community social and support services for the continued care of individuals.

Diversion Agreement Monitoring

  • Monitors diverted or deferred individuals compliance with the approved jail diversion plan and reports status to Director of Community Integration and appropriate Prosecutor

Data Collection and Reporting

  • Process CareLogic reports daily, check jail log, and communicate the days needs to the treatment team.
  • Identify Jail Diversion options and track reporting data in collaboration with Jail Mental Health Professional, Crisis Intervention Team, and CC Program Evaluation.
  • Provide monthly updates on diversion activity to North Central Director

Locates hospital beds for DCR functions or as needed in ITA investigation / detention

Provides Transitional Case Manager under the direction of the Diversion Program Team Lead

Jail Services Screening

  • Upon referrals from Jail Mental Health Professional, Completes Jail Requests for Services and makes appropriate referral for jail intake and transition services provided by Catholic Charities.

Completes necessary paperwork as required, meeting established timelines and making authorized entries in the medical record; follows all procedures for release of information and maintains complete and accurate, concise records and reports

Fosters teamwork and cooperation between team members

  • Works to understand others on the team and is a cooperative team member who willingly helps others perform related jobs

Identifies and prioritizes issues, problems and concerns that need to be addressed and resolved

  • Works to facilitate resolution accordingly including others in the decision making process when appropriate to make independent, effective decisions using available information.

Maintains confidentiality in accordance with HIPAA regulations and RCW 70.02.230, and educates individuals on matters requiring informed consent.

  • Participates in supervision with Director of Community Integration and multi-disciplinary team staff meetings, training, specialist and psychiatric consultation as indicated/requested and required by supervisor

Hospital Diversion and Transitional Care Support Responsibilities

  • ITA - Detention Follow up
  • As assigned, meets with individuals on 72 hour and 14 days Involuntary Treatment at Confluence Health Medical Unit One
  • Attend and adhere to communication procedures for rounds
  • Completes Columbia Suicide Severity Rating Scale, Mini Mental State Exams, and other assessment procedures identified in Joint CFCS MU-1 Operations manual.
  • Completes consult notes or other paperwork daily as assigned
  • Maintain professional, courteous communication with all levels of staff.
  • Rehabilitative Case Management
  • Assess MU-1 client readiness for discharge, provide brief, solution focused therapy with the goal of developing discharge and service plans designed to prevent premature readmission to hospital.
  • Works with Catholic Charities and other Providers to support creative utilization of community resources; requests staff use of specific assistance as needed to support management of individuals with mental illness in the community.
  • Reinforces and supports advance planning and creative utilization of resources to stabilize individuals in Crisis Stabilization or Shelters and reduce length of hospital stays.
  • Less Restrictive Setting
  • Supports use of engagement and creative problem-solving with individuals and their families in order to facilitate the least restrictive placement that meets the consumers and communitys needs.

Coordinates with Hospital Liaison to ensure coordination with agency staff and the agencys Crisis Services

Works closely with the North Central Director, Hospital Liaison and DCRs to ensure appropriate care alternatives are identified and implemented whenever possible to achieve client stabilization and decrease frequency and length of hospital stays to the extent possible.

  • Locates Hospital Beds
  • Contact psychiatric hospitals, E&Ts, to coordinate transfer of clients to a higher level of care as needed
  • Present case histories, coordinate requested assessments, documentation, hospital transfer paperwork
  • Communicate and monitor regarding waitlist information

Supports and models engagement with individuals with serious mental illness

Encourages staff members use of appropriate treatment options and recovery supports to address and remove potential obstacles to client recovery.

Model assertive outreach ethic for Crisis team members

Ensures the implementation of agencys Mental Health guidelines and procedures for individuals served in the crisis system

Advocates for persons served when the need exists

Promotes and teaches advocacy skills to staff when needed

Performs other duties as assigned including backup DCR if so designated

Candidate is expected to become designated within 6 months

Job Requirements:

This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision.

Non-Physical Requirements:

Education

  • Associate or Bachelor'sDegree in Social Behavioral Sciences or minimum of 1 year field experience in intensive case management and/or jail diversion services, preferred
  • High School Diploma or GED, Required

Experience

  • Minimum of one years experience in the provision of Mental Health Case Management Services

Special Skills

  • Knowledge of Behavioral Health Care delivery to adults and children
  • Knowledge of Jail Diversion processes and procedures, including consent and confidentiality
  • Ability to work with others to accomplish objectives and achieve expected results
  • Knowledge of DCR Protocols
  • Strong crisis assessment and stabilization skills
  • Possess good verbal and written communication skills for communicating with persons served of all ages and staff
  • Uses the computer effectively and adheres to Catholic Charities standards following the Agencys security guidelines and procedures

Licensure, Registration, Certification

  • Agency Affiliated Required and actively working toward completion of degree or other Case Manager Training / Certification
  • Valid Washington State drivers license, access to vehicle andminimum required liability insurance for WA State
  • Must be deemed insurable as determined by Catholic Charities' liability insurance provider

Employment is conditional upon:

  • Being cleared by criminal background check and fingerprinting when required
