RN Care Transitions Coordinator
Job Summary: Provides education as well as oversight and acts as a liaison with the care transitions team, patients, physicians, families, other health care personnel, hospital administration, external agencies and community groups. Ensures best practices for the quality of transitional care services. Responsible for performing job duties in accordance with mission, vision, and values of Tampa General Hospital. Required Skills: Bachelor's Degree Nursing License/Registration - Issued by Florida or Compact State - Registered Nurse. 3 years related work experience required; BSN may be completed within 2 years of hire from an accredited institution. Skill in monitoring/assessing and supervising the performance of other individuals.
