Job Description

The primary responsibility of your job position is to provide direct nursing care to villagers, and to follow the current federal, state and local standards, guidelines and regulations that govern long term care. You are entrusted to ensure that the highest degree of quality care can be provided to our villagers at all times. Working collaboratively with the villagers, Village Care Provider, and the villagers' family develop and follow a plan of care that meets the individual goals of each villager. Nurses in Bluestem Village model operate as:

KEY RESPONSIBILITIES:

  • Coordinate villager care with other members of the clinical support team and the Village Care Provider (CNA).
  • Obtain an update on each villager from the Village Care Provider (CNA) during each shift
  • Visit each villager during each shift to monitor clinical status, and to determine if the clinical care needs of the Villager are being met. Clinical assessments should be villager-specific and consider the primary diagnosis and risks specific to the individual villager
  • Assess each villager for unresolved/untreated pain. Plan and implement pain management interventions when appropriate.
  • Assess, plan and implement interventions related to: skin integrity, bowel and bladder status, including incontinence and bowel movement monitoring, mentation, ambulation status, nutrition (weights) and hydration, and the effects of medication
  • Identify any clinical teaching needs of the Village Care Provider (CNA), villager or family. Provide teaching and complete documentation
  • Identify ways to support Village Care Provider in their clinical competency and skills
  • Work various houses or work flexible schedules as agency and supervisor deems necessary. Staff attendance and promptness to assigned shifts is essential. The responsibilities of this position may require staff to be able to work irregular, extended, and/or be available for on-call work hours, evenings, nights, weekends, holidays, etc.
  • Monitor standard precautions (i.e. presence of gloves, soap, towels, etc.)
  • Administer treatments and medications
  • Monitor overall adequacy of the care plan
  • Review compliance with the care plan
  • Maintain emergency supplies and equipment
  • Conduct physical assessment of each villager
  • Conduct MDS assessments, monitor infection control, and care planning activities
  • Work in collaboration with the Village Care Provider (CNA's) Teams, the villagers, and their family to ensure the highest level of well-being.
  • Provide villager care according to physician orders.
  • Communicate changes in villagers' condition.
  • Perform administrative duties such as: completing medical forms, reports, evaluations, studies, charting, etc.
  • Admit, transfer, and discharge villagers as required.
  • Complete accident/incident reports.
  • Assist Clinical Coordinator with budgetary development and control.
  • Inventory and order necessary clinical supplies for your houses.
  • Coordinate adequate pharmaceutical supplies with consulting pharmacy.
  • Respond to family concerns and complaints.
  • Establish open lines of communication between all employees.
  • Assure that villager abuse does not occur.
  • Respond to all villagers call requests regardless of assignment.
  • Complete wound care rounds to assess, develop, implement, and evaluate various types wound care needs on each villager.
  • Provide wound care to medically complex villagers in collaboration with the physician.
  • May act in the absence of the Clinical Coordinator or Support Team
  • Maintain confidentiality and rights of villagers, family, employee, and Bluestem Village information in compliance with PHI HIPAA policies and applicable regulations.
  • Maintains safe work environment and exhibits safe work practices.
  • Attend care plan meetings as assigned.
  • Demonstrate a passion for caring as evidenced by interaction with co-workers, villagers, families, and visitors.
  • Follow agency procedures when purchasing items on behalf of the agency.
  • Follow agency policies and procedures in carrying out duties.
  • Advise supervisor of concerns and problems that arise within the agency.
  • Safeguard all Bluestem Village funds and property, including those individuals in our care.
  • Performs other duties as assigned by the Supervisor, Administrator, and/or designee.
  • Perform all other duties, as assigned.

SKILLS AND KNOWLEDGE:

  • Knowledge of computer systems including Point Click Care
  • Working knowledge of long-term care and skilled nursing state and federal regulations (preferred)
  • Knowledge of nursing and medical practices, procedures and standards of care
  • Leadership and supervisory skills-willingness to work harmoniously with and supervise other personnel
  • Patience, tact and a cheerful disposition, enthusiasm as well as willingness to handle difficult people
  • Knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served



EDUCATION AND EXPERIENCE:

Experience: One-year experience in a long-term care environment (preferred)

License: Current, unencumbered, active license to practice as RN in Colorado, CPR Certification



PHYSICAL REQUIREMENTS:

  • Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times, difficult people within the community
  • Meet the general health requirements set forth by the policies of the community which include a mental and physical examination
  • Possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met
  • Ability to push, pull, move and/or lift a minimum of 50 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum of 50 feet
  • Ability to assist in the evacuation of residents during emergency situations

WORK ENVIRONMENT:

  • Is subject to difficult and emotionally upset villagers family members, personnel, visitors etc.
  • Move intermittently during working hours
  • Subject to frequent interruptions
  • Involved with villagers, personnel, visitors, government agencies etc. under all conditions and circumstances
  • Subject to difficult and emotionally upset villagers, family members etc.
  • Work beyond normal duty hours on weekends and in other positions temporarily when necessary
  • Subject to call back during emergency conditions (e.g. severe weather, evacuation, post-disaster etc.)
  • Subject to injury from falls, burns, odors etc., throughout the workday as well as reactions from dust, disinfectants and other air contaminants
  • Subject to exposure to infectious waste, diseases, conditions etc., including but not limited to TB, AIDS and Hepatitis B viruses
  • May be subject to handling of and exposure to hazardous chemicals
