The Mental Health Association of Columbia-Greene Counties Inc. is looking to fill the position of Childrens Case Manager. This position is full time 37.5 hours per week, benefit eligible and the pay range is $17-20 per/hr. This is a School Based position working with both Ichabod Crane and Taconic Hills School Districts.

You’ll get to be part of a dynamic, diverse, and supportive behavioral health organization whose mission is to "Provide education and advocacy, and to enhance the wellbeing of individuals, families and communities."

This program consists of three services: School-Based Programs (SBPP), Community-Based Prevention Program (CBPP) and Supervision and Treatment Services for Juveniles Program (STSJP) Hudson and Columbia County. The Ichabod Crane and Taconic Hills School-Based Prevention Programs are collaborative efforts between the Columbia County DSS, the two school districts and MHACGC. Based at the school, MHACGC staff provides services to families with children who have serious emotional, social and/or behavioral issues that put them at risk of being placed in out-of-home settings or dropping out of or failing in school. The staff offers a variety of supports to enable the family to keep the child at home and the family intact. CBPP assists children with serious emotional disturbances, provides supports to families and links families to appropriate services in the community. The goal of CBPP is to prevent out-of-home placements by providing case management support to families throughout the two counties. The program works with the entire family not just the identified child.

Job duties include:

  • Carries a caseload of 12 families and accepts referrals from primary to middle school building principals, the Department of Social Services, Probation Department and the Single Point of Access.
  • Has frequent, consistent contact, preferably in the family’s home with the youth, family and/or significant others.
  • Provides advocacy services by assisting with family specific needs related to other systems of care and acts as program contact person.
  • Coordinates meetings with the families among service providers and school personnel when needed and/or appropriate.
  • Provides information in a timely manner to the Case Management Program Director related to units of service, number of families served, admissions and discharges, and any other reporting requirements (Board of Director Reports, time sheets, mileage sheets, etc.).
  • Attends training related to family empowerment, multi-cultural sensitivity, case management, among others.
  • Will be required to participate in our Strengthening Families Parenting Program as required by our grant with the Department of Social Services.
  • Will participate in 4 sessions of Strengthening Families Program per year during the evening.
  • Assist in a facilitation role, support staff, daycare, or transportation role as agreed upon by you and the program supervisor.


Requirements:


  • Must have a bachelors degree or at least 3 years of experience working with families in the Human Services Field.
  • Must have knowledge of Children and Families services in Columbia and Greene County.
  • Must have knowledge of Department of Social Services and the operations of School Districts.
  • Understanding of cultural and family differences and values, and ability to remain non-judgmental is essential.
  • Ability to work with a diverse population of urban, suburban and rural cultures and with community members from varying socioeconomic backgrounds.
  • Demonstrate a commitment to Diversity, Equity, and Inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias.
  • Must be able to work regularly outside the office and in the field to engage with clients.
  • Excellent verbal, written, communication, and interpersonal skills.
  • Bi-lingual a plus
