Support Coordinator I

Support Coordinator position with a starting salary range of $40,000 - $42,600.

Education:  A bachelor's degree required.

Description:  beginning-level professional case management work in a comprehensive purchase of service and community placement program for developmentally disabled individuals.

An employee in this class is responsible for managing a caseload of moderate size and complexity.  As a member of a multi-disciplinary treatment team, the employee participates and confers with others in determining the proper treatment to purchase or procure for individuals.  Work includes contacts with various community agencies serving the developmentally disabled, and may, on a limited basis, involve responsibility for conducting specialized individual assessments.  Supervision is received from a Case Management Supervisor.

Coordinates information and input with the participant, family, guardian, and/or service provides, regarding the development of the Individual Support Plans (ISPs) on an annual basis and initially when beginning MCSDD Support Coordination.  Ensuring that the Individual Support Plans are completed timely.  Complete ISP amendments in a timely manner, as needed throughout the ISP year.  Link resources to services.
