CoreLife has created an organized, convenient, and low-cost multidisciplinary platform of care for those who so often fall through the cracks of today’s highly fragmented healthcare delivery system. We enhance total health by providing a multifaceted approach to obesity, chronic illnesses, the root causes, social determinants, and related side effects.

Position Description:

The Nurse Practitioner provides clinical care to patients through an interdisciplinary model, serving as the lead member of a care team consisting also of a dietitian, exercise trainer, and other clinicians that may be utilized in the model.  The Nurse Practitioner will monitor and manage the problems and conditions associated with obesity, prescribe pharmacotherapy to treat obesity, and engage patients in the multidisciplinary service model offered by CoreLife. This position will be a float position for our Montgomery County clinics: Germantown and Silver Spring.

Major Areas of Responsibility: 

  • Introduce patients to CoreLife's philosophy and multifaceted pathway to improved health
  • Assist patients in creating a specific goal for weight reduction and health improvement including a target body mass index.
  • Obtain vital signs, pulse oximetry, and body composition
  • Obtain Medical/Surgical/Family/Social History
  • Perform a comprehensive review of systems & physical exam including all major organ systems 
  • Reviewing, Ordering, and Interpreting relevant laboratory data.
  • Draw blood for testing, when needed
  • Work within a multidisciplinary clinical team consisting of a registered dietitian and exercise trainer, among others
  • Prescribe pharmacotherapy, including phentermine, as an adjunct to the CoreLife multidisciplinary model to treat obesity
  • Establish a problem list for each new patient that details all of the current medical conditions associated with obesity & all of the potential conditions that could develop based on risk factors and family history, and execute a care plan over a series of follow up visits that serve to manage & mitigate these conditions
  • Perform preventative counseling, behavioral counseling, and other services at each visit
  • Enroll in and prescribe appropriate patients all the services provided by CoreLife including the use of the CoreLife app, remote patient monitoring, and therapeutic exercise, among others
  • Timely and accurate documentation in an EMR of the patient encounter and all services provided during a visit
  • Initiate bidirectional communication with the referring provider with relevant events or changes in condition during the patient lifecycle

Other Responsibilities: 

  • Explain how CoreLife uses a Team Approach to achieve improved health. Explain each aspect of medical/nutrition/exercise components that are of equal value
  • Explain the clinical model of an initial encounter with a medical provider, and subsequent visits with a medical provider, or their designee, and the registered dietitian on a weekly, biweekly, or other frequency based on an individualized patient care plan.  Adding additional visits with other team members based on an individualized care plan
  • Document and complete all components of the office visit within 24 hours.

Education:

  • Minimum requirement of a Master’s Degree in Nursing from an accredited institution

Work Experience:

  • Minimum requirement of 2 years of direct patient care experience in office or hospital-based capacity as a nurse practitioner
  • Experience must include evaluating, monitoring and documenting issues related to chronic medical conditions, especially those associated with obesity

Licensure/Certification/Affiliation:

  • Licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
  • Certification as a Nurse Practitioner by the AANP or ANCC
  • Board Certified Nurse Practitioner
  • Appropriate state of licensure and clinical certification, DEA and Controlled Substance License in practicing state.  
  • Current CPR

  • Critical thinking in a patient care environment
  • Strong comfortability in medical decision making and prescribing medication
  • Interpersonal skills that establish productive working relationships with a multidisciplinary team and administrative support
  • High level ability to use an electronic medical record
  • Ability to communicate effectively in both written and verbal form to patients, public, medical staff, and physicians
  • Creating a patient care environment that is sensitive to individuals suffering from obesity
  • Ability to counsel and motivate others, as individuals and as a group.
  • Experience working in a diverse environment
  • Flexibility to adapt to changes in medical protocols and policy.
  • Accepting educational advice, coaching, and novel approaches to patient care as delivered by the medical leadership team

Work Environment:

This job operates in a professional medical office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms.
