Join our Neighbor Services team as a Healthcare Specialist! This is a temporary role.

The Neighbor Services Healthcare Specialist is responsible for creating and maintaining relationships with healthcare partners, with the goal of increasing enrollment and providing excellent customer service and support throughout the program. Click here to learn more about the FoodRx program.

Additionally, this candidate will provide information and assistance to people potentially eligible for the FOODRx program and other programs Second Harvest Heartland (SHH) offers, such as the Supplemental Nutrition Assistance Program (SNAP) and the Commodity Supplemental Food Program (CSFP).

Starting Pay Range: $21 - $23 USD hourly / Dependent on Qualifications and Experience.

Work Schedule:

  • Full-Time, 40 hours / week.
  • Hours: Monday – Friday: 8:00am – 4:30pm
  • Hybrid work schedule – minimum of 2-3 days per week in-office
  • Duration: 6 months

Duties and Responsibilities:

  • Partner with healthcare organizations, community partners, individuals, and others to deliver FOODRx services.
  • Lead and maintain partnerships with healthcare organizations. This includes scheduling and holding regular meetings with partners, providing reporting, acting as an internal subject matter expert on the partnership(s), and other duties as assigned by the Supervisor.
  • Provide excellent customer service for healthcare partners, patients and their families, and other stakeholders.
  • Support Neighbor Services Department with neighbor requests as needed. Connect with neighbors who are referred to SHH for services or directly from healthcare partners.
  • Connect patients to appropriate community resources and/or assist with enrollment for all Neighbor Services’ programs.
  • Support Neighbor Services Department to conduct initial eligibility screenings and engagement calls for FOODRx, and for SNAP and CSFP, as needed.
  • Identify and develop strategies to overcome barriers that prevent patients from fully utilizing FOODRx program.
  • Collect required data and provide timely, accurate and complete reports that support informed decision making and overall attainment of programmatic outcomes.
  • Review data from healthcare partners for accuracy and completeness. Provide additional training and support as needed to ensure high quality data collection.
  • Preserve patient confidentiality by adhering to HIPAA guidelines.
  • Continually identify, develop, and test business processes and new opportunities for FOODRx and Neighbor Services team.

Must Have:

  • A combination of education and experience generally attained through a bachelor’s degree in a related field(s) (social services, nutrition, public health, etc.) and/or 2-4 years’ experience in the areas of health care, customer service, sales, or program enrollment preferred.
  • Valid MN drivers license and legally mandated insurance.
  • Access to internet connection during entire workday when working off-site. Connection with speed of at least 80 GB internet, with 100 GB preferred. Also, a hardwired connection is preferred over wi-fi.
  • When off-site, must have the ability to work confidentially and with privacy, which may include a separate space with a door.
  • When off-site, must have the ability to secure workspace and equipment to prevent loss, damage, or breach of security.
  • Experience working independently and as part of a team with diverse populations.
  • Comfortable speaking on the phone for long periods of time, communicating through email and text, and talking face-to-face with others.
  • Strong time management skills.
  • Demonstrated ability to effectively communicate with professional English skills in written and oral forms.


Nice to Have:

  • Experience working with healthcare and community based/nonprofit organizations.
  • Ability to speak a second language highly desired and experience working with diverse populations. (Spanish, Somali, Hmong).
  • Training or supervisory experience (volunteers, interns, etc.)
  • Interest in health equity, nutrition, food access, and/or other social determinants of health.
