Job Description

The Peer Specialist Case Manager functions as part of a multidisciplinary team as someone who has experience as a recipient of mental health services for severe and persistent mental illness and is willing to use and share his or her personal, practical experience, knowledge, and first-hand insight to benefit the team and its clients. Peer Specialist Case Manager follows-up with referrals made by the Prosecutorial Diversion Program’s Care Coordinators and Mental Health Professional.   Provide ongoing case management services to persons experiencing symptoms of acute or chronic mental illness until such time as a primary therapist, medication management provider, and/or case manager can be assigned from one of the community mental health centers.  Provide case management services for adults who have chronic mental illnesses or severe mental disorders in jail, detox, crisis triage facilities, and psychiatric inpatient facilities.  Provides evaluations, designs, implements, and coordinates services in order to assist the person in transitioning from jail, detox, crisis triage facilities, and psychiatric inpatient facilities to the community in order to maintain an optimum living environment and to decrease the barriers to functional community living. Peer Specialist Case Manager will assist with obtaining benefits, housing assistance, and supported employment services. Coordination of care will include but not be limited to; medication management, substance use treatment, therapy, housing, physical health needs, developmental needs, identification and partnering with natural supports and vocational needs as deemed appropriate.

Shift is 20 hours per week, 4 hours per day.

\nQUALIFICATIONS\n
    \n
  1. Education: High School Diploma or GED required
  2. \n
  3. Population Specific: Knowledge and skills necessary to provide care appropriate to the population of patients served
  4. \n
  5. Experience: Experience assisting persons, who have experience mental health problems, self-identify as a recipient or survivor of mental health services and/or co-occurring, well grounded in recovery for a minimum of one (1) year required
  6. \n
  7. Certification:  Have or obtain a current Certified Washington State Peer Counselor within 12 months of employment in position. Have or obtain a current Washington State Agency Affiliated Counselor within 12 months of employment date.

  9. Required Licensure: Must have/obtain a current Washington State Driver’s License with Liability Insurance, to use personal vehicle
  10. \n
  11. Other: Bilingual and/or English-Spanish-Speaking ability preferred. Ability to work with a culturally diverse population Employee will possess a cell phone to be used at work and a stipend will be given to cover this expense
